What Do My Lab Test Results Indicate?
Question: Sir in october 2014,me and my wife got treatment in vdrl.My titer was 1:160 and my wife titer was 1:80. After four month of treatment (penicillin 24 lac power three shot in three weeks and erythromycine 500 mg four time for 15 days) in April 2015 my titer showed nonrective and my wife titer was 1:1.But in august 2015 again my titer shows 1:320 and my wife titer shows 1:80.I confused what is th problem. This time in march 2016 my wife is 8 month pregnant kindly tell me if again we have to take the treatment.
Brief Answer:
Syphilis -serologic follow up
Detailed Answer:
There are two possibilities. One is treatment failure and another is reinfection.
Since both of you showed initial serological response following standard treatment ,reinfection is most likely.
In any case since VDRL titre is rising again ,hence re-treatment is warranted for both of you. What about HIV status regarding any co-infection?

Sir one year crossed taking my treatment so can we take treatment again this time or give sometime to see what is titer in coming time because doctor is saying it take one or two year in decreaing the titer. And my wife titer is constant as before treatment 1:80.After for month of treatment it was 1:1.And again after 4 month of first test it was 1:80.This time she was pregnant 8 month what should do.
Brief Answer:
Syphilis -serologic follow up
Detailed Answer:
After successful treatment , the VDRL titer progressively declines,
becoming negative by 12 months in 40–75% of sero-positive primary
cases and in 20–40% of secondary cases but in persons with latent or late syphilis it may take 24 months. However rise in titre after gap of one year is altogether different matter.
Secondly congenital syphilis of fetus to untreated maternal infection is another concern.
A second opinion from another venereologist would be most welcome.
