Brief Answer:
Symptoms are related to your irregular cycle...
Detailed Answer:
Thank for the question and I am happy to help.
As you may know, uterine fibroids are extremely common. They are benign tumors of the uterus that arise from smooth muscle cells. They are associated with heavy painful periods. This may explain some of your pelvic discomfort, but your question has to do with irregular cycles and symptoms similar to pregnancy.
In most cases such as yours, the problem has to do with irregular ovulation. Counting the 1st day of your cycle as day #1, most women ovulate on day #14, and get their cycle on day #28. When you do not ovulate or ovulation is delayed, there is irregularity in terms of when your cycle occurs or if it occurs at all. This is consistent with what you are describing. In addition, comment made on ultrasound of multiple ovarian cysts is possibly consistent with a syndrome called polycystic ovarian syndrome. When you do not ovulate, cysts that normally go away towards the 2nd half of your cycle persist and make the ovaries look polycystic.
Also, when your cycle is delayed, the typical symptoms of breast tenderness fatigue nausea etc. are exaggerated and mimic pregnancy. What you are describing is extremely common and often in the setting of continued negative pregnancy tests. Causes of delayed or absent ovulation include:
1. Being underweight or overweight
2. Thyroid disease
3. Elevated prolactin levels
4. Diabetes or pre-diabetes
If you are trying to get pregnant, then you need evaluation for the above listed problems. If you are not trying to get pregnant, then you may need hormones to regulate your cycle.
With regard to the pain in the pelvis, as stated, this might be related to the fibroids, but it might also be related to ovarian cysts related to not ovulating.
Please let me know if this helps so far.
