Question: I have red circlular marks around the inside of my legs, sort of large. Went to my nurse pracititioner yesterday. She is treating it like a yeast infection Gave my a Fluconazole 150 mg, Also have Nystalin and Triamcinolone Acetonide ointment. Not as angry as it once was, never had anything like this. Did have a small tear on my anus a couple of weeks ago.. Also use Cottonelle wipes. Have swithched to Charmin.. I do have very sensitive skin. She thought it resembled ring worm with the circular shape. They are not blistering, just scary. Any thoughts? Did do a urine sample. No bacteria, but do show some blood in my urine. Not the first time for that. Thanks you, XXXXXXX XXXXXXX
Brief Answer:
Use an antifungal cream & treat fomites too.

Hi there XXXXXXX XXXXXXX
I have gone through your history & description of the rash.
I would have to say, at the outset, I tend to agree with your nurse' assessment.
It does appear to be a circinate tinea cruris or simply put a 'fungal infection'. However, I disagree yeast are responsible; it appears more like a 'dermatophyte' infection. Use of topical steroids like triamcinolone may have masked the lesion but helped decrease the 'angry' red look.
Please switch to a cream containing 2%ketoconazole commonly available the world over as Nizoral. Apply this twice a day over the next week or two & the rash should clear up. Another dose of fluconazole after a week is possibly just what the doc ordered & all will be well.
A foot note: please ensure all your undergarments' & towels etc are washed in hot water & dried well to keep the fungal elements at bay.
Good Luck & Godspeed!
