What Do The Blood Sugar Level Readings Indicate?
To monitor the sugar level he uses accu chek machine.
And it reads as follows :
Before Meal i.e fasting : 125
two after having a meal (poha and sujji halwa(sweet) ) : 115
The pattern we noticed is that sugar level reading is always higher for fasting than Post meal.
Usually people sugar level increases post meal however here it is reverse and it holds true every time test is done.
Highly appreciated if all of my queries are answered :
1. why is it so ? (sugar level increases post meal)
2. Is there anything to be worried about ?
3. What checks/diagnosis are required ?
4. What is the remedy ?
5. To whom i need to visit ?
6. Medicine taking to control sugar level is correct or not ?
7.Shall he continue with the same medicine or need to visit the doctor or you prescribe any medicine ?
8.Is it normal to be found diabetics at this age i.e is it common ?
9.Whether accu chek machine reading is reliable to know the exact sugar level or there are differences with the actual sugar level (test done at diagnostic center)
10. What should be ideal sugar level before and after meal for my Father.
Note :
My father walks in morning daily and also post meal whenever possible.
My father parents are not diabetics.
My Father sister is diagnosed with diabetes.
Consult diabetologist/endocrinologist.
Detailed Answer:
Hi,
Thanks for your question.
Noted your concern.
One by one answers to your questions-
1) Diabetes is due to insulin resistance (opposition to action of insulin).
Insulin resistance usually affect liver (which is responsible for mainly fasting blood sugar level) and peripheral tissues like muscles (which is responsible for mainly postprandial blood sugar level.
Difference in insulin resistance at two different level is responsible for higher fasting than postprandial blood sugar level.
2) At present his blood sugar levels are under control. So no worry. But diabetic education is important.
3) At the time of diagnosis with age of 62, I usually suggest following investigations-
Complete Blood Count
HbA1c
Renal function Test
Liver Function Test
Urine examination.
Ultra TSH
ECG
2DEcho of heart
4) Diet restriction, regular exercise as tolerated, medications and regular follow ups with treating doctor.
5, 6, 7) Treatment may need to be modified depending upon his physical examination and above test results.
For which it may be advisable to consult diabetologist or endocrinologist.
Also consult near by dietician who can plan diet for him considering his BMI (Body Mass Index) and food habits.
8) In general diabetes incidence is increasing world wide.
9) Glucometer reading help in guiding blood sugar control, it doesn't give absolute reading. It show variation in blood sugar levels when compared with laboratory values.
10) Considering his recent detection of diabetes and age his Fasting blood sugar should be below 120 and postprandial below 180.
Hope this helps him.

Dr. Abhay Mali.
