Brief Answer:
Endometrial biopsy should be done

Detailed Answer:
Hi dear, I have gone through your question and understand your concerns.I have reviewed your case, your lab reports and your discussion with Dr XXXXXXX and Dr XXXXXXX
All your lab reports are normal except Pap smear which shows inflammatory smear, and is due to infection, and needs antibiotic treatment. But it is not the cause of irregular and prolonged bleeding.
You should also get your ultrasound done now, as only spotting is there now and it does not interfere with the ultrasound report.
You are most likely having hormonal disturbances which are causing irregular bleeding and prolonged spotting from the uterus. The hormonal disturbances lead to changes in the uterine lining (endometrium) which then sheds irregularly for a prolonged time.
Such a condition needs endometrial biopsy to diagnose dysfunction uterine bleeding, endometrial hyperplasia or rarely endometrial carcinoma, all of which cause irregular and heavy bleeding. After making a definitive diagnosis, further treatment should be initiated.
Endometrial hyperplasia and dysfunction uterine bleeding are common in patients of PCOD, and since you have such history, endometrial biopsy is advisable in your case. It is a simple OPD procedure, and can be performed by any gynecologist.
