What Do The Following Liver Function Test Reports Indicate?

default
Posted on Fri, 28 Oct 2016
Question: Hi,

I have been suffering from Hepatitis like problem. Initial tests have shown that Hepatitis A, B, C & E are negative. However in initial Liver Functioning Test the Billirubin and SGOT/SGPT were on high end. Below are the values:
S. BILIRUBIN 1.9 (direct 1.0; indirect 0.9)
SGOT 3250
SGPT 4430

After medication, the values of SGOT and SGPT went down to 462 and 1870 respectively. However, the BILIRUBIN is increasing. In latest LFT test, 4.5 was the value (3.5 direct; 1.0 indirect).

- Can you please suggest correct course of future actions to be taken in this scenario?
- Can Hepatitis A&E be detected in early stage of disease?
- Any other expert comment on above health report will be welcome.

I can send the exact medical reports if needed.

Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

The liver enzymes are very high and is common with viral hepatitis.
But this is also see due to certain medications, herbal remedies and autoimmune hepatitis.

I presume you are not on any liver toxic medications or herbal remedies.
Please upload the following reports -

1. Tests pertaining to Hepatitis A, B, C and E
2. PT/INR
3. Ultrasound scan of the abdomen

Hi Sir,

Thanks for reply.

There was no herbal remedies being done at my end.

I had fever on Sunday last week; so I consulted a general physician. I was prescribed Paracetamol. This helped to bring down the fever. However, after three days symptoms such as Nausea, vomiting, low blood pressure and abdomen pain started to appear.

Hence I consulted doctor in a private clinic. I got admitted in hospital and getting medication as I attached with main query.

I have attached ultrasound and tests for Hepatitis.

Hi Sir,

I do not have PT/INR report, since the test is not done yet.

One more LFT test was done today and it shows that BILIRUBIN has reduced now (by 1.6) and also SGOT/SGPT decreasing.

Brief Answer:
Answer given below

Detailed Answer:
Hi,
Since the tests for Hepatitis A,B, C and E are negative, the increased liver enzymes are due to other causes like HSV or EBV or other viruses ( since it is of recent onset).

Get your LFT checked every week.
Take Silybon 140mg twice a day.

If the liver enzymes are not returning to baseline or if it is worsening, tests for causes other than viral infection may be done (which is mentioned in my first post).

Regards,
Dr.R.K.
