What Do These 6-TGN And Fecal Calprotectin Levels Indicate?
Question: Hi
I have a question about my daughter who is 11 and has Crohn's. She has been on Imuran since July (50mg initially, then 25mg one day 50 mg the next since Sept). Her 6TGN levels are now very high (1300) but fecal calceprotein is at 313. Does this indicate she is refractory and we need to change drugs?

Brief Answer:
Needs other markers
Detailed Answer:
Hi
Thanks for writing to us.
There are a number of markers to predict inflammation or severity in crohns disease. Fecal calprotectin is one good marker. More than 100 indicates inflammation. If this value is decreasing then your child is probably responding to treatment which is fine.
6TGN doesn't indicate disease severity. It is a marker of drug metabolism which latest studies say may not be useful even for that.
Enquire about other markers like CRP XXXXXXX etc which are widely recommended markers to assess treatment response.
Wishing your child speedy recovery.
