What Do These LFT And Ultrasound Reports Indicate?
Question: Hi,
I started to loose my appetite 1 month ago with a dull back pain on right side back and underwent LFT and ultrasound. Ultrasound indicated fatty liver- grade one. LFT indicated a bilirubin level(1.54 mg/dl - direct 0.54 and indirect 1.0), SGPT(ALT) 68, SGOT(AST) 34, ALP(99). My GP recommended udiliv 150mg twice a day and liv.52 for 1 week. After that, I stopped medication for 2 days and went for LFT again whose results came out to be - bilirubin level(1.18 mg/dl - direct 0.42 and indirect .7), SGPT(ALT) 53, SGOT(AST) 38, ALP(87), GGT(29). Again, I continued udiliv 150 for 5 days, then udiliv 300 for another 2 days. I underwent ultrasound and LFT again. As usual, US indicated fatty liver grade one. However LFT read - bilirubin 1.5 mg/dl(direct-.5 indirect - 1.0), SGPT(ALT) 79, SGOT(AST) 53, ALP(85), GGT(22). Rest all tests read in normal range. Please suggest/advise. I am 33 years old male weighing 76 kgs

I have carefully gone through the entire query. I felt the second ultrasound and LFT meaninglessly done. Rather, there are a couple of things to check first. The first thing to ask is whether you take alcohol. The next thing is to ask you to get your blood sugar (fasting and post prandial) checked. Alcohol is a known cause of fatty liver although another entity called non-alcohilic steatohepatitis (fatty changes of liver) or XXXXXXX is also known. There is some linkage of XXXXXXX with increased blood glucose levels.

Thanks for the response Dr. As far as alcohol is concerned, I have been a regular drinker, that too was weekly once. I stopped taking alcohol more than one month ago that was 4 days prior to my first ultrasound and LFT test. In my last test, I got blood sugar level 92(fasting). I did not go for post prandial check though.

Just abstain from alcohol and things would be back to normal.

Thanks for writing back. Stopping alcohol completely should gradually help in resolution of the fatty changes in liver. The fasting level is otherwise fine and since the alcoholism is otherwise there, I do not think that post prandial level is required.
