What Do These MRI Findings Indicate?

Question: Hi there my MRI shows osteodsical protrusion with osteadiscal bar formation between C3 and C6
At C3/4 there is Central osteodsical protrusion bar formation slightly eccentric to the right .no impingement upon cord or canal facet joints are unremarkable
At C4/5 there's broad based posterior annular protrusion eccentric to the left with impingement upon the left C5neural exit and nerve .the right c5neural exit is unremarkable
At C5/6 there is a let sided eccentric disc protrusion wth osteadiscal bar formation
There is severe impingement upon the left C6 neural exit and nerve
This is the most marked and striking finding on this study
Question
I was trying to lift a very heavy frame with one other person- me at the front holding it on my left side with right arm across my body to the left holding the Frame and leading.the person behind me dropped the frame at the back.I twisted my head quickly to try and see what had happened .The frame almost fell .i held on to it while twisting my neck as well.somehting happened to my neck then and there
The next day or So aside from shoulder pain,I get the nerve pain running down my arm to my fingers.the next week or so pins and needles,numbness,major shooting nerve pain etc
My family doctor claims the MRI results are not due to this injury but we're pre existing.i find this hard to believe ,when all my pain and symptoms started after this injury
I have been thought a lot of pain and discomfort since then
It's been 12 months since injury
MRI findings are 12 months after injury
Could the MRI findings -ot part of be a result of this injury?
What do you recommend is next step for me after this MRI result ?

I am Dr.Ajay Panwar,a neurologist,here to answer your query.

I have gone through your MRI cervical spine report.Here,my opinion tends to differ from your family doctor.Though it is possible,that mild disc protrusions could have pre-existed before injury.However,at the same time,such an injury can very well increase the severity of disc protrusion.In fact,it can even result in disc herniation.So,this injury could have contributed to increased severity of disc protrusion(like the one at C5-C6 in your case which is the most prominent one and causing impingement upon the exiting nerve root),thus resulting in your symptoms.

In any case,whether the changes were pre-existing or whether injury resulted in these changes,the treatment plan remains the same.Medicines and Physiotherapy(cervical spine exercises) are the mainstay of the therapy.
You should be on a treatment protocol,typically comprising of-
1)Antineuralgic medicines-like Pregabalin,Amitriptyline or Duloxetine
2)Methylcobalamin supplementation
3)Calcium/vitamin D supplementation

Along with,regular dedicated cervical spine exercises are extremely important.Please learn exercises technique(correct technique is the key to improvement) from an expert physiotherapist and do these exercises regularly.

Thank you for your prompt response
Aside from advising me as to treatment from hereon , you have also supported my suspicions all along that the pain and symptoms may be due to the injury
This injury occurred at work.i made a work cover claim but it was rejected due to this doctors report.
Now I am more confident in asking for a reevaluation of my injury
I have suffered greatly in the last 12 months because of the nerve pain and pins and needles
You have clarified to me the situation now
Also your treatment is more appealing to me than my doctors suggestion for neurosurgery ,which I want to avoid
After two steroid injection to the spine-which I didn't want to do,but was given no other choice,To relieve the pain , I want to be certain of what treatment I will follow next before I proceed
If you tell me all this pain and discomfort may be due to my injury,I can now proceed with confidence
And I can do what I need to do legally and medically to get justice for me

Epidural steroids injections may be used if symptoms do not respond.

Of course,you should move ahead on treatment protocol with medicines and physiotherapy.Epidural steroids injections may be used if your pain is not responding to medicines and physiotherapy.You are,apparently,functionally independent and so,this,still,is not probably the time for surgery.
