What Do These Bilirubin And SGOT Values Indicate?
I am 30 Male having Liver disorders. My Bilirubin is not normal and sometimes SGOT and SGPT also increase. I suffered from acute pancreatitis due to alcohol consumption in Feb 2008 but after that I didn't take any alcohol and the liver reports also came normal but always my bilirubin levels are slightly increased before pancreatitis issue also my bilirubin levels were slightly high .
Just to inform you that sometimes SGOT and SGPT is normal but bilirubin slightly increased.
I was diagnosed with Typhoid recently and at that time the LFT report was also increased and I took Udliv 300mg, pantaprazole and Cefixime and typhoid become Negative but still the Bilirubin is not normal. Can you please advice the diet and further treatment.
Below is the report detail when I was diagnosed with Typhoid and after that the latest reports are attached herewith along with one old report which was done 3 months back..
S.Bilirubin 3.56, Direct Bilirubin 2.10, Indirect Bilirubin 1.46, SGOt 103, SGPT 122, Alkalaine phospate 143, Typhidot IgM Positive.
Bil levels are expected to come down in 2-3 weeks.

Hi Sir.
I understand your query and concern and gone through your reports.
You have recent bilirubin of 2.63 which is not that high to be over concerned.
It will come down to normal levels in 2-3 weeks.
Acute pancreatitis and Typhoid can lead to sustained elevations of Sr.Bilirubin ,but is expected to resolve.
Diet should predominantly consist of Green leafy vegetables and Oatmeal of 50 gm taken in 100 ml of milk daily.
Include fruits like Pomegranate and apples daily in your diet.
Avoid fried foods strictly until the bilirubin resolves.
You have so far had very good treatment .Hence complete biochemical cure will soon likely to happen.
In case if the bilirubin levels does not completely resolve in 4 weeks ,i advise you to have an ERCP done to rule out occult gall stones.

Actually the issue is that I was diagnosed with typhoid recently and this may be the reason for high bilirubin but I have observed that even if I go for random LFT Test then in that case also the Bilirubin was high even though the SGOT/SGPT will be around 40 but the Bilirubin levels are in the range of 3-4. So what may be the possible reasons. I have some old reports that I can share with you and just to inform you that I was diagnosed with pancreatitis in Feb 2008 and its more than 6 Years and the bilirubin levels are random. Also during that treatment in 2008 I was tested for endoscopy, CT Scan and MRI for the whole abdomen and all the reports were normal and nothing abnormal observed and the possiblity of stone was also ruled out during that time.
So is it normal to have high bilirubin and normal SGOT/SGPT..??
What could be the outcome of physical activity and stress in this scenario.?? Is working in the night shift also effect the Liver conditions..?? Will it deteriorate the condition if the other LFT functions normal except bilirubin..??And kindly also suggest about the food in this scenario and treatment.

Gall stone sludge could be responsible.

Hi Sir.
The cause of recurrent elevations of bilirubin in your case seem to occult microlithiasis/tiny gall stones.
This has most probably lead to the acute pancreatitis in 2008.
Most cases of acute pancreatitis resolve spontaneously.But the gall stone sludge tend to appear periodically causing obstruction to bile flow leading to asymptomatic elevations of serum bilirubin.
In all such cases you can have normal SGPT and SGOT.
Working overnights can impose metabolic stress on liver,thereby deteriorating its function if continued for long duration.Try to avoid it if possible.
As earlier stated its extremely important to avoid fried and fatty food.Oatmeal and pomegranate are very good sources in liver derangements.
