What Do These Following Blood And Urine Reports Indicate?
Question: My 65 old mom has severe blood sugar recently.She is diabetic for last 10 years.
And medcine that she is taking is
gluformin (1-0-1).
The values are given below
Blood Sugar After 2hours having food:
29 july 2016 : 360
1 aug 2016 : 450
3 aug 2016 : 325
4 aug 2016 : 220
10 aug 2016: 269
11 aug 2016: 353
12 aug 2016: 378
Fast:
28 july 2016: 345
5 aug 2016 : 250
6 aug 2016: 234
I need to know which insulin can be given/medicines. Moreover I know how to give insulin and check sugar level with glucometer.
Also she has urine infection as per lab report taken yesterday. I have attached the lab report.

Bacteria present
Epithelial cell 0-1
Rbcs 0-1
Urine sugar yellow
urine albumin yrace
urine micro albumin 72.89
haemoglobin 13.4
Blood examination
Blood sugar 353 mg/ dL
Serum creatine 0.5 mg/dL
Serum total chlestrol 126 mg/ dL
Please advice medicines for this urine infection and insulin treatment for diabetic.

TSH (checks your thyroid)
Urine or Serum Ketones
25 hydroxy Vitamin D levels (ideal range 40 to 60 ng/ml = 100 to 150 nmol/liter)
