Be happy that both Pap -smear and endometrial biopsy are negative, it means that they have excluded any serious issue as endometrial cancer or cervical cancer that can cause irregular vaginal bleeding .
In my experience as a Gynecologist I have treated patient with irregular vaginal bleeding due to use of Plan -B.
Hence in my opinion you should be calm, eat healthy especially iron rich food and drink more fluids (water).
While your blood work shows that your hormones are really good you can't say that you have PCOS. PCOS diagnosis is needed to have hormonal dis-balances.
About 40 % of women have polycystic ovaries but don't have PCOS.

Here are my bleeding dates since plan B consumption (3 separate times took Plan B in July-Aug)
-Aug 31st - Sep 9th = PERIOD (10 days)
-Sep 10th - Sep 17th = NO PERIOD (8 days)
-Sept 18th - NOW = PERIOD (18 days) :(
I have been eating so super healthy and been doing CrossFit three times a week. I lost 160 pounds and have been maintaining the weight loss for 2 years now and lost the weight and the course of 2.5 years prior to that. I'm 30 yrs old.
Questions:
1. What are my treatment options after seeing my bleeding dates above? I've been on my period for 18 days straight now! :( Sometimes it pauses for a few hours and changes colors and sometines clots. How long do you expect it to last
2. Do you agree that I should not feel pressured to take birth control of all my hormones are good? Why would I want to disturb or something that's good? I've been birth control for you for almost 8 years now. Maybe that's why Plan B had such a dramatic effect on my cycle. Your thoughts?
3. Do I need treatment for polycystic ovaries and what are the side effects?
You were the smartest person that I've talk to you when I've been to four docs so far. Thank you so much!

I will try to give you a better answer for every question you have made.
1.Treatment vary form quantity and duration of vaginal bleeding, your general state(how you feel).
You can discuss with your doctor to start Vitamin K injection for minimally 5 days while you don't want to start hormones .
2. If vaginal bleeding is abundant and you doesn't feel better you can take birth control pills even if your hormones are within range because they will help to stop vaginal bleeding , you should take minimally for one month.
As I've written above, I have had patients like you , that plan B have caused abundant bleeding .
3. You don't have PCOS because all your hormones are within range and you don't have any signs and symptoms to justify this diagnose .

I have had patient with irregular vaginal bleeding form the use of Plan B but when it was prolonged and it caused anemia,then I have prescribed treatment.
I usually begin with Vit K injection, and if it doesn't work I have prescribed other treatment as hormonal therapy.
Hence in my opinion if you continue with abundant vaginal bleeding you should contact with your doctor and begin with a therapy.
