What Do These Following Lab Reports Indicate?

Posted on Sat, 2 Jan 2016
Question: Hello Doctor,
Couple of days back I started having pain in my lower abdomen (LHS Below waist line), and I thought it could be gastric and went to the hospital and got couple of injections and got the relief.
But, I started having similar pain yesterday again and I went to a physician who prescribed Ultrasound , CUE and UricAcid test. Urine and blood test seems normal but ultra sound shows presence of calculus (8mm) and significant post void residual volume.
I am feeling frequent urge of urination (after every 15-20mins) and urine comes with slight pain and having discomfort at the tip of my discharge point.
Also, I've been experiencing frequent urination problem from last few months but without any noticeable problem. Problem started from 3-4 days only i.e discomfort and too frequent urination with mild pain
I had this stone problem couple of years back as well and it came out from my body unnoticeably since it was very small in size
This time it seems bigger and am worried whether it will come out on it own.
I've attached my reports for your reference.
Please provide medication for relief/ further course of action
Thanks

Detailed Answer:
Hello
Thanks for your query ,based on the facts and ultrasound scanning films and report that you have posted it appears that you have a stone of a size of 8 mm in lower ureter on left side .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have vomiting ,Fever associated with pain
2) Is pain tolerable or very severe ?
3) Do you have to strain while passing urine ?
4) How is your urine stream whether poor, moderate or normal?

The stone that you have is impacted in intramural portion of ureter which is narrowest part of where it enters in to bladder.

However you can expect it to pass down in bladder .Follw foollowing guide lines
1) Drink more water
2) Take Tomsulosin once daily
3) Take Drotavarine twice daily ,both these drugs relax smooth muscles in ureter and dilate the ureter and help to pass the stone down into bladder without pain .

With this treatment it will pass down in bladder within a week ,

However in a worst scenario if the pain becomes very severe and you get vomiting ,fever which suggest infection you may need to unedrgo endoscopic surgery of Ureterorenoscopy(URS) to remove/fragment the stone .

Please note that the size of the stone is bigger (8MM) hence it is likely to get impacted in urethra giving severe pain in penis while passing out,

As regards your significant post void residual urine it needs to be evaluated to rule out stricture of the urethra (Narrowing of urethral passage ).

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.
