What Do These Following Lab Reports Indicate?

Question: Hello again. I can't remember if you were the doctor that said I could post the results of the bloodtests when I got them. But I do to day.
I was also at cardiologist because of the heart-related issues me and you have discussed earlier here. I took a ultrasound, a cycling test and EKG. It was all fine, with an incomplete block right side. And low bloodpressure. Because I told him I get chestpain, he said: Since you are uncertain and have anxiety for this, I will send you to do an CT of the veins. I am scared because of the contrast used, which I have reacted on earlier doing a scintigraphy.
Anyway here are some results. Its in norwegian so not sure about all of it is easy for you to read.
Elevated ACTH anti "drug" against the enzyme 21-hydroksylase is a marker for autoimmune - and I try to translate: adrenal destruction. Not kidney, but in norwegian bi-kidneys. Values were 13.1 and should be under 11,6.
Low vit D. Values: 26, and should be 50-113.
b12: 246. S-Folate: 25.4. P-homocystein:12,8. Estimated GFR: 110. S-metylmalonacid: 0,17. S-kortisol:430. S11-deoxycort:1,3. S-17-OH.prog:4,6. S-DHEA-sulfate:8,5. S-aldosteron: 179. P-renin consentrate: 23,2. S-25 hydr.Vit D3: 26. P-metanefrin: 0,25. P-normetanefrin: ok. S-210H anti: 1,1.

I carefully reviewed all your uploaded tests and would like to have some more information on the units behind each value (ex.: mmol/dl, ng/dl, etc.), because all these values depend on the type of unit behind them.

Could you please make a photo of your performed tests and upload them, because it could be more easy to determine all these values from the lab report.

Regarding your vitamin D plasma levels, you should know that there are different reference ranges, based on the lab where these tests are performed. Many experts recommend a level between 20 and 40 ng/mL. Others recommend a level between 30 and 50 ng/mL. So values of 26 can be considered borderline.

Anyway, considering the fact that you live in Norvey (where there is not enough sunlight), I would recommend starting taking vitamin D supplements for this purpose.

Regarding the other tests, they could indicate an adrenal gland (which is positioned over the kidneys) dysfunction.

At your age, congenital disorders are excluded (because they are usually diagnosed since childhood).

Coming to this point, I would recommend consulting with an endocrinologist in order to determine the type of hormonal imbalance and the proper treatment therapy.

You should know that adrenal gland insufficiency can lead to such clinical scenario. Anyway, I can not determine correctly these values, without their specified units, in order to come into more specific medical conclusions.

Hi dear doc.

I have just uploaded the testresults.
The value should be between 50-113 i think.

Reassuring that nothing was found on ekg while cycling today, right? Even if I feel a "pressure" on the chest.

Yes adrenal gland is the word I was looking for.

1. Vitamin D levels are low based on this lab normal ranges. This means that you need to take vitamin D supplements for a long time, until these levels will be normal again.

2. ACTH levels are slightly high. 21-Hydroxylase antibodies have resulted high too. They are markers of autoimmune XXXXXXX disease, which is adrenal gland insufficiency. Although, your cortisol plasma levels have resulted normal.

All these tests seem to be indicative for the beginning of possible adrenal gland insufficiency. I would recommend consulting with an endocrinologist for this purpose and discuss the proper treatment options.

You should know that all your symptoms could be explained by this disorder. That is why it should be treated properly by a good specialist of this field.

My doctor has ordered me to take divisun d3 vitamins 800 ie, and b12 methyl sub tablets.
He also say that we need to take another set of blood to confirm, so then I guess I will have to see a specialist? So I can relax on the clogged arteries that i fear, and heartfailure after cardiologist even if I have press in chest- this might be the reason why? The low D and other things on bloodtests? You have no idea how good to hear.
