What Do These Following Lab Reports Indicate?
ECochG (Electrocochlearography)
Detailed Answer:
Hi,
Thank you for writing back.
1. It is a little early to reach a conclusion.
2. An ECochG (Electrocohlearography) is used where there is a persistent failed OAE test and no ABR response.
3. The study of Cochlear Microphonics (CM) is done, If the CM are large, this lack of hearing may be due to ANSD (Audiotory Neiropathy Spectrum Disorder). A study of the SP (Summating Potential) and the AP (Action Potential) also helps.
4. In ANSD, half the children are normal but mature later. Of the remaining, a further half of these children do well with hearing aids. The rest do well with cochlear implants.
5. The ECochG can be done with a Tympanic Electrode under general anesthesia. If there is no response, a Transtympanic electrode can be used under general anesthesia.
6. Kindly upload these results.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
Regards.
Audiometry in KEM hospital asked me to do a follow up after 8 weeks, but he said its unlikely the results will change. Should we conduct this ECochG Electrocochlearography test soon now or should I wait till we do a follow up test after 8 weeks.
The news of ABR failing has totally devastated my family and me. I was wondering in worst case if the results continue to be the way it is now can my baby lead an normal life going to a normal school. I am very much worried about her future. I thought of meeting you yesterday at KEM hospital but after this test I couldn't handle it and went back home.

Detailed Answer:
Hi,
Thank you for writing back.
1. You should wait for 8 to 10 weeks before further tests.
2. A hearing aid trial can be useful if the ABR continues to show decreased hearing.
3. It is possible to manage with or without hearing. Many people do well with hearing aids which have advanced technology now. In the worst case scenario, a cochlear implant will help her lead a near normal life.
I hope that I have answered your query. If you have any more questions I will be available to answer them.
