What Do These Ultrasound And Blood Reports Indicate?

Question: Hi Doctor,

During health checkup of my mother , aged 57 years old, following observations were done ( I am quoting only abnormal results). Rest were normal.

Ultrasound - mild spleen enlarged - 13 cm.

CBC WITH PERIPHERERAL SMEAR, ESR AND EDTA -

Haemoglobin 11.4 range - 12-15
MCH 25.8 range 27-32
MCHC 30.6 range 31.5- 34.5
RDW - 17 range 11.6 - 14

LYMPHOCYTES - 17.4 range 20-40

ESR ( WESTERGREEN) -22, range <20

Blood picture :
rbc - normocytic normochromic
wbc - count within normal range
platelet adequate

LFT :
ALKALINE PHOSPHATASE 148 range 40-129
GGT 6 range 8 -37

She is on diabetes medicine Jalra M and tyhronorm 50. Her sugar level is well within control.Her physical condition is good. no fever, loss of weight, or any abnormal symptoms.

I am worried with these findings. Is there something that needs to be done here. Please advise.
Somewhere on internet I read about enlarged spleen and leukemia and this has made me worried.

No chance of leukemia so don't worry about it.

Hi, dear
I have gone through your question. I can understand your concern.

She has mild enlargement of spleen, but her WBC count is normal so there is no chance of leukemia. It can be due to some infection.

She has low MCH, MCHC and slightly high RDW. It is commonly seen in iron deficiency so she may have iron deficiency anemia. She should check her iron level once and if needed she can take iron supplements.

Her lymphocytes count is low but not significant. If she has high neutrophils then it may be due to some infection.

Slight high esr is not a cause of concern.

Her peripheral smear study is normal.

Her alkaline phosphatase is slightly but its not significant in absent of other findings. Low GGT has nothing to worry.

So overall there may be some infection and iron deficiency. No any major cause is there. So nothing to worry about that.

If she has any symptoms then please tell me about that.

I can understand your concern.

You should not worry about leukemia. Spleen enlargement is associated with leukemia, but there are so many other non serious causes.

In leukemia total blood count is very high with abnormal cells in blood picture. Your blood picture is absolutely normal and its no way near about leukemia. So don't worry about leukemia. Just be relaxed.
