Thank you for posting on HCM.
I have reviewed your picture and I can think of two possibilities:
1. Vitiligo
2. Post inflammatory hypopigmentation.
Its difficult to point specific diagnosis in absence of clinical examination.
Therefore, I insist you to mention following points in history:
1. Duration
2. History of any skin lesion over these sites before you noticed the white patches and if so please describe what kind lesions they were.
3. Any family history.
4. Any history of chronic contact with chemical or use of any steroid based cream over the sites.
5. Any history of trauma over the sites.
Apart from history also try to post good clinical pictures of lesions over scrotum and other sites, so that I can help you in a better way.

Case History:
1. Duration: I have these spots for two years now. Initially,they appeared as small spots and gradually grew big.
2. History of any skin lesion over these sites before you noticed the white patches and if so please describe what kind lesions they were:
No lesion were there on these spots before these spots appeared. They are now smooth discolored not white as such but slightly pink with no pain or not even sticky at all.
3. Any family history: No one from my paternal or Maternal side have had any history of skin discolor.
4. Any history of chronic contact with chemical or use of any steroid based cream over the sites:
I have only used Quadriderm Cream in the scrotum area before for itching. After consulting the doctor I have been using Pimecrolimus Cream PACROMA.
5. Any history of trauma over the sites:
No physical injury on these places before these white spots appeared.

Oral levamizole, other topical preparation

Hello,
Thank you for the detailed history.
After keen review, I would like to treat you for vitiligo on primary basis with post inflammatory hypopigmentation as differential.
Just using Pimecrolimus (Pacroma) wont be enough.
If I was your treating doctor, I would start you up with oral levamizole 150mg one two consecutive days of week. Levamizole is an immunomodulator drug that has very good response in vitiligo.
Also I would suggest application of mometasone cream ( ELOCON cream) in morning and tacrolimus 0.1% ointment at night time.
I would consider reviewing my treatment after 1 month and there is no satisfactory response, I will consider addition of oral corticosteroids and other topical medications like fibroblast growth factor lotion etc.
But for noe I would like you to just start te basic treatment I advised and update me your condition after 1 month.

Thanks for advice. I could not understand the dose of oral levamizole 150mg. Rest creams related doses i could make out. Please confirm the dose of oral levamizole 150mg. I ll update in a months time. Thanks.

Detailed Answer:
Levamizole tablet of 150 mg should be taken once a day on any two consecutive days in a week.
For example you can choose to take one tablet on every Monday and Tuesday after lunch and then a gap of 5 days and then again next Monday and Tuesday take medications and so on.
Do this for 4-5 weeks and then report me back.
