Question: I have report for HSG which states
Both the tubes are blocked at its fimbrial end with no evidence of peritoneal spillage seen on either side.
Would you please let me know what does it mean ?

HSG is a test to check whether the fallopian tubes are patent. Thus is done usually for patients who want to conceive and have difficulty getting pregnant.
The fallopian tube connect the uterus to ovary.
The sperms have to swim from the vagina through the uterus and reach the fallopian tube.
The ovary releases eggs which are received by the fimbrial end of the tubes.
Finally fertilization i.e egg meets sperm and a fetus(baby) is produced.
If the test shows that fimbrial end is blocked then it means that the sperms have no access to the eggs.
Thus there may be difficulty in conceiving.

Dear,
In cases like yours where the tubes look blocked on HSG require further analysis by diagnostic laparoscopy.
This is a minor surgical procedure where under anaesthesia a laparoscope or camera is placed inside your abdomen through a small incision in umbilicus and a colour XXXXXXX is pushed through the vagina and uterus similar to HSG.
The exact location of the block in your tube is better visualised in laparoscopy.
Besides fimbrial adhesions which may be there in your case as seen in hsg , they can be treated so that block is released and tubes become patent. Any other minor tube pathology is easily treated.
But the success rate and pregnancy depends on the damage to the fallopian tubes.
If on laparoscopy the fallopian tubes have major damage, extensive blocks or infection, then IVF or invitro fertilisation is the only option as extensive tubal damage cannot be treated completely.
I hope your other tests are normal as blood parameters, hormone analysis, ovarian follicle study and husbands semen analysis should be normal for you to conceive.
