
What Does "Normal Size Heterogeneous Gland With 2 Right Sided Nodules" Mean?

EXAM: Thryoid/Soft Tissue Neck Ultrasound
History: Nodule
Findings: No prior thyroid imaging studies for comparsion. The thyroid gland size appears normal right love 5 cm left lobe 4.7 cm. The gland has a moderately heterogeneous texture bilaterally. There is a cystic nodule medial mid right lobe 9mm diameter and a heterogeneous lower pole right nodule slightly echogenic 16mm.
Impression: Normal size heterogeneous gland with 2 right sided nodules. Since this is a baseline study for this patient consider follow-up study in 4-6 months to assess stability.
Not suggestive of carcinoma.

This USG report is suggestive of MULTI NODULAR GOITRE,and nothing to suggest carcinoma thyroid.
With such a report,usually biopsy is not advised.In thyroid nodule,we go for biopsy/FNAC if it is a solitary nodule(here there are two nodules).
Anyhow,need for biopsy can not be determined based solely on the USG report.It also need consideration of the detailed clinical profile.Features favoring a biopsy include pain/recent increase in size/a hard thyroid on palpation/associated lymph node enlargement/family history of thyroid cancer etc.
If there is nothing like these,as the radiologist advised,we just need follow up and a repeat USG after few months if needed.
