Cheesy thing is smegma & there may be phimosis
Detailed Answer:
Hi,
Welcome to HCM ...
You said that cheesy thing is accumulated underneath foreskin of penis. That causes irritation. And retraction of foreskin is difficult..right?
I can understand your worry about the condition of your penile foreskin and cheesy material causing irritation and itching.
Cheesy material is smegma. It is produced by glands in prepucial skin. It should be cleaned daily. If you do not clean it, it may remain there between glans and prepuce. And that might cause irritation and discomfort. Ultimately you might get infection...balanoposthitis. If you have difficulty in retracting your prepuce.......(foreskin) ,that may be the condition called phimosis.
Please consult dermatologist or surgeon immediately. Let him to see the condition of your penis. He might clean the area with antiseptic. And the collected smegma may be removed. Antibiotic cream may be advised to apply. If he feels that your foreskin is not retracted in normal way and glans of penis is difficult to see..then it may be phimosis and he might advise a surgery called circumcision. It is a minor surgery done under local anaesthesia in which the foreskin is removed. And the glans of penis is completely remained opened. Your problem would be resolved by this operation.
I hope this would help you... Still if you have any query, you are welcome.
Best wishes..
Thanks.

Hi,
After reading history you provided .. I would recommend you to consult nearby dermatologist to have firm diagnosis. After examining you, he would decide whether you require circumcision or not? Please follow his advice for further management to resolve your problem.
I do not know where you reside, so it is difficult for me to suggest any dermatologist.
Thanks..
