It is hard to know the cause of heart defects. Sometimes they occur from random genetic mutations and sometimes they are an inherited defect. If your other two girls are healthy then that points to a random mutation in your child that died. One way to get a better sense of what happened to the girl that died is to have a doctor look at the medical records from her chart in XXXXXXX and tell you what they think was the cause.
