Question: I have a non painful mobile 2 to 3 mm nodule at the left base of my penis. It moves freely with the skin. It developed within 16 hours of oral sex from my wife of 28 years. I have shaved my penis shaft and scrotum for years now. It does not appear to be an ingrown hair. Totally pain free and very hard like a little pellet. What are the possible causes of this very rapidly appearing nodule?

Thanks for your query,based on the facts that you have posted it appears that you have what is called as Sebaceous Cyst .However final diagnosis can be confirmed only on clinical examination by a qualified General Surgeon.
I shall be happy if you can post me few more details to help me in making comments on your problem.
1) What is the duration of lump?
2) Can you see punctum (White spot on the summit of lump)?
3) Is there any whitish cheesy discharge from the lump?
The skin around the base of the penis is rich in sebaceous glands and hence more prone to get more sebaceous cyst due to accumulation of sebum beneath the skin.
Normally they fade away without treatment however they need to be treated if increase in size or get infected.
Please consult qualified General Surgeon for clinical evaluation and further treatment.
If needed it may need to get excised in Toto (Completely along with the sac).

I am a board certified anesthesiologist of 25 years. I have seen sebaceous cysts before. It would seem they would take some time to develop. This nodule is very firm almost rock like. No punctum and no discharge. There does not appear to be any pore or surface orifice or any follicle involved. I noticed its appearance from nothing to a quite noticeable nodule essentially overnight. I can easily pinch the entire nodule between thumb and finger and there is no compression... Very firm. What can develop this fast? I guess I want to know if I am safe to just observe if there is no change in size at it remains painless. It appears rather benign and may have decreased in size just a little over the last 24 hours. It has only been present for about 36 hours.

Small benign painless ,firm lump developed within 36 hours after oral sex and gradually subsiding can be a localised haematoma and can be observed for couple of days and expected to regress of it's own .Final diagnosis can be confirmed only on clinical examination .

I am not aware of any STD that would present this way. Are there any reasonable concerns that I should give to a STD presenting as a nodule beneath the skin as described above? I HAVE had protected sex outside of marriage recently. Only oral sex was unprotected.

It is not STD at all.
It could be a localised haematoma due to trivial trauma during unprotected oral sex and can be confirmed only on direct inspection and palpation of a nodule by a qualified Surgeon.
