The results of your test are inconclusive, if you had a positive pregnancy test reading in the past. The presence of a gestational sac is usually by 5 weeks if the dates are absolutely right. Sometimes it may appear a bit later by 6 weeks or so.
However, the absence of the gestational sac means that there might be delayed development or chances of a miscarriage. You may be called for a follow up scan at 6 weeks and still no sac found indicates a missed abortion.
Also ectopic pregnancy should be ruled out if there is no sac and your urine pregnancy test is positive. Rising hCG levels means there could be a ectopic pregnancy but falling levels might mean that you may have a miscarriage.
It would be better to have a follow up scan after 6 weeks gestation and if required few other investigations might be required. Please see your gynecologist as the treatment depends according to the results.

The endometrial thickness depends on which part of the cycle the scan was done. Yours is not abnormal but in pregnancy its usually found to be thick.
It would be better to repeat the scan or do beta hCG levels. This will indicate whether you are pregnant or not. Also you need to rule out ectopic if your urine pregnancy test was positive.

Yes UPT was positive and confirmed by doctor also on date 30 may 2011. what should be done to rule out Ectopic

Ectopic pregnancy would be ruled out in the Ultrasound scan of the Abdomen. If there is no mention then it would be better to get a repeat the scan at 6 weeks.
