albumin to creatinine ratio is useful when measured in the urine, so it has to be a urine test. The units are also very important. If you'd upload the report it would make it much easier for me to give you a definite answer.
If the units are mg/gr (milligram/gram) then 490 is a very high value indicating kidney disease.

Other useful information include your serum creatinine and urea level and also your albumin/total protein levels, if you've done these tests.
