What Does Amniotic Fluid Index Of 22 During 25 Weeks Of Pregnancy?
I am 25 years first time pregnant women who have completed 25 weeks of pregnancy. Recently it is found I have a polyhydraminos with afi level of 22cm. My doctor said I may have premature baby and I should go on complete bed rest. My placenta is in an anterior position with grade 1 level. I am really scared since this is my first pregnancy. I am worried about premature baby at any time from now. Kindly advice me about my polyhydraminos and anterior placenta. I am feeling my kicks often at my vaginal area. So I am really scared . Kindly guide me. Please

Hi dear, I have gone through your question and understand your concerns. An amniotic fluid index( AFI) of 22 definitely means that there is increased fluid around the fetus, but it is not Polyhydramnios, as it is said to occur when the AFI is more than25. The cause of Polyhydramnios can be gestation diabetes mellitus, any congenital anomaly or infection in the fetus or idiopathic( cause not found). The cause must be found out and treated in your case. Increased fluid can lead to preterm labour and delivery, and bed rest do not help in reducing fluid or preventing preterm labour, so not advisable. Rather you should restrict physical activities and avoid heavy work.
I would suggest you to get your blood sugar levels, ultrasound to detect fetal anomalies, blood tests to check TORCH infection in order to find the cause for increased fluid around the fetus.
In case no cause is found, you should get follow up ultrasound every 2 weekly to monitor the fluid levels. If it increases beyond normal levels, you can be given medications to reduce the fluid.
Feeling kicking movements in the vaginal area is normal in pregnancy.
Anterior grade 1 placenta is completely normal.
I would suggest you not to worry and get properly treated by an OBGYN specialist.

Hi dear, thanks for responding. If all the tests are normal, then there is no reason to worry. You should get2 weekly ultrasound done, and if the fluid increases beyond 25, then you can be given indomethacin drug to reduce the fluid.
Moreover, if the fluid remains at same level or increases, you should then be given steroid injection( betamethasone) at 28-32 weeks to accelerate fetal lung maturity, to avoid the risk of respiratory distress if there is a premature delivery. AFI of 22 is not extremely increased, and hence don't worry so much. Excessive stress will affect the growth of the fetus and can lead to preterm labour and delivery.

Preterm labour before 37 weeks of gestation

Hi dear, thanks for responding. The fluid do not get reduced, but there is no problem if it does not increases with time.
Preterm labour is said to occur when the labour occurs before37 weeks of gestation. It cannot be predicted from now that when will it occur.
If you have leakage of amniotic fluid, you will feel a gush of warm fluid coming out from your vagina, and clothes will get wet with that clear fluid.
