What Does An Itchy White Patch Inside Anus Indicate?

Possibilities discussed
Detailed Answer:
hi, welcome to HCM.
You need to rule out syphilis. It is still not uncommon in homosexuals. It presents as painless, firm, non itchy papule which leads to ulceration and bleeding.
As you have mentioned initial white patch which were itchy, there may be possibility of tinea cruris (a fungal infection), if you can send clinical photograph to us, then it will be better for diagnosis.
There is a feature to upload the image by yourself on the right side of this query page. You may utilize that. Alternatively, you can mail it to YYYY@YYYY with subject line as "Attention to Dr. Shahid Hassan".
Wart is a small, rough, cauliflower like growth called condyloma accuminata.
Since you are applying nystatin, fungal infection will subside and not lead to bumps.
Hemorrhoids and warts will take time to develop and not just in one or two week time.
serum VDRL and serum TPHA will rule out or confirm syphilis.
For how many weeks before you noticed white patch?
Thank you

both candidal infection and sentinel pile,no bumps due to fungal infection
Detailed Answer:
You have got two diseases as clearly shown in the picture.
First one (the bump) is sentinel pile, it is a circumscribed thickening of mucous membrane at the lower end of fissure of anus. it generally occurs after long stool, constipation, rectal examination or anal intercourse. This started with anal intercourse. Was this painful at the start?
Initially this presents with stinging and burning pain during bowel movements, itching and bleeding.
Ointments which contains anesthetics, steroids, nitroglycerin, and calcium channel blocking drugs are used to treating this. Before this stool softener and high fiber diet should be taken which helps in healing.
Second one is (White patch) is Candidal Intertrigo.
It needs sertaconazole cream for local application and cap Isotretinoin 100 mg twice daily for seven days for healing. sertaconazole may be available over the counter for you but Isotretinoin is a prescription drug. Please discuss with your doctor before taking the medicine.
Keep the area clean and dry for proper healing and prevent recurrence.
Hope this is helpful.
