Brief Answer:
Needs evaluation

Detailed Answer:
Hi,
When an ultrasound is performed by the sonologists, they expect the kidney to have a smooth outline with a demarcation of two distinct layers of the kidney known as cortex and medulla.
When this differentiation is not clear or when the outline is irregular, it means there could be a kidney damage.
The way to know if this is happening by having a basic kidney profile done. This includes testing complete blood picture, blood urea, serum creatinine and electrolytes, and a urine analysis. If there is any feature of infection, urine culture can be done.
Treatment will have to be specific for the cause.
If the investigations reveal no biochemical issue and if there is no infection, you can wait and watch and have a review scan done after 3 months.
In the meantime, please take a reasonable amount if oral fluids and restrict salt in diet. Take a regular physical exercise.
Once the reports are available we will be in a position to advice further.
Regards.

Brief Answer:
Less salt, regular check, fluid intake moderate

Detailed Answer:
Dear Mrs XXXXXXX
Looks like your creatinine value is just about normal, while the electrolyte levels are also within normal limits.
As of now, I doubt if you need to do anything aggressive.
I suggest you get an opinion of a nephrologist as well. I am not aware if you have blood pressure, but it is important you get that regularly checked, and keep under control. Restrict your salt intake as I earlier mentioned. Keep monitoring your creatinine and urine analysis every 3 monthly, and act upon any slightest hint of a urinary infection.
There are other tests which might be required later if it looks like the kidney echogenicity is increasing or the size is coming down. Some of the tests include urine for micro-albumin, MCUG which is a special X-ray, and nuclear scan in select patients.
As of now, things do not look bad, so please be reassured.
Regards.
