Thanks for your query ,based on the facts that you have posted it appears that you have dull pain in testicles and painful ejaculation with ED since last 6 months .
I shall be happy if you can post me few more details to help me in making comments on your problem.
1)Do you have urinary symptoms like frequency for urination ,nocturnal frequency ,urgency or hesitancy suggestive of enlarged prostate which is common at this age?
2) Are you diabetic?
3) Are you taking any medications ?
Symptoms that you have are suggestive of infection of Prostate ( Prostatitis) and needs to be confirmed
Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis

1) Yes, I do have some urinary symptoms - 2-3 times waking up at night for urination, some urgency, but most annoying/prominent is the constant dull aching (feels like my testicles but perhaps radiating from my prostate/base of penis).
2) Not diabetic as far as I know but my father was (Type 2). Interestingly I am experiencing some symptoms similar to diabetes (eg, tingling/pain in feet/toes) but I think I would have the other symptoms as well, don't you? Maybe just bad circulation? I am fairly active and not overweight (188 lbs, 5'10") but don't get much aerobic exercise
3) Not taking any medications
A couple questions:
1. Assume I am diagnosed by a urologist with Prostatisis and the anti-biotics work, is it likely to recur? Ie, how do things down there even get infected and is there anything I can do to avoid recurrence?
2. Is Doxicycline over the counter in the U.S., or would I need a prescription?
3. Can I get Cialis by e-commerce/mail in the U.S. (safely) or do I need a prescription? Is there a safe website you recommend?
4. So it sounds like I shouldn't be too worried about prostate cancer at this point, more than likely an infection//inflammation

Prostatitis is a notorious condition for patient as well as for treating Dr.
which is known to have relapses and recurrence .One has to manage on antibiotis as per culture report
2) As regards your query about Diabetes youin view of your father being diabetic there are 50% chance of you to be diabetic .
Get yor blood sugar tests done to rule out te same .
3) Prostate cancer be ruled out by digital rectal examination by qualified Urologist and by doing a basic screening test of PSA.
4) Cialis is available ion online market in USA and should not face any problem in getting it .
