default
Posted on Tue, 1 Apr 2014
Question: Hi...m 33 weeks pregnant......today i did ultrasound.....my AFI is 14.......found single loose loop around babys neck....n low blood flow to babby....plz help.....m worried...hw shud i overcome it???dr hs given me two betnesol steroid injection....cn dat cause lower abdominal pain??
doctor
Answered by Dr. Sree Gouri SR (1 hour later)
Brief Answer: Bed rest in left lateral position, l arginine sach Detailed Answer: Hi, Thanks for the query. Sometimes single loop of umbilical cord can be seen around the neck of the baby. Usually loose loop may not cause problems. Better to be careful, count fetal movements daily. If you find decreased fetal movements, consult gynecologist. Your AFI is normal. Decreased blood supply to the baby may be indicating increased vascular resistance. This can be seen in conditions like hypertension, gestational diabetes etc. Measures like bed rest preferably in left lateral position, taking l-arginine sachets etc can improve the blood flow to some extent. Usually corticosteroids injection itself may not lead to pain abdomen. Better to get examined once to rule out the possibility of premature uterine contractions. If your doctor suggests go for progesterone supplementation. Hope I have answered your query. I will be available to answer followup queries. If you are satisfied with the answer, please give your review at the end of the discussion. Take care.

Thank u so much....m feeling less baby movements...bt dr is saying its coz of steroid shots....which i took yday n day bfore....wat shud i do to avoid preterm birth..as m jst 33weeks....m sending u my letest us report.... BPD :8.0CM,HEAD CIR:29.0,ABD CIR:27.9FEMUR LNGH:6.4CM,FETAL WT :1920GM...... UMBILICAL ARTERY....RI:0.8...PI 1.4....s/d 4.7 Middle cerebral artery RI.....0.8 PI 2.3 Early fetoplacental inefficiency n single loose loop of cord arnd fetal neck
doctor
Answered by Dr. Sree Gouri SR (1 hour later)
Brief Answer: early fetoplacental insufficiency Detailed Answer: Hi, Steroid shots may not affect the fetal movements. You calculate fetal movements for one hour after breakfast, one hour after lunch and one hour after dinner. If around 10 to 15 fetal movements occur in these three hours, that can be considered as normal. According to your ultrasound report, AC is little less suggesting possibility of intrauterine growth retardation. Umbilical artery RI, S/D ratio are little bit high suggesting the possibility of early fetoplacental insufficiency. But reference values can differ from lab to lab. By taking measures like bed rest in left lateral position etc which I mentioned previously condition can be improved. Progesterone supplementation can help to some extent in preventing the preterm labor. Take care.

Answered by Dr. Sree Gouri SR (2 hours later)
Brief Answer: Nutritious diet, adequate rest Detailed Answer: Hi, Hald Sr 200 contains hydroxyprogesterone caproate. It belongs to category D drugs in pregnancy. So, better to use natural micronized progesterones like naturogest, susten etc as per as possible. By taking sufficient nutrious diet, bed rest, adequate water, supplements like l arginine etc, nutrients and blood supply to the baby can be increased. This can help in preventing intrauterine growth retardation to some extent. Placental insufficiency can cause decreased blood supply to the baby. This can occur due to increased vascular resistance, aging of placenta etc. This can lead to intrauterine growth retardation, oilgohydramnios etc problems. But by proper care these can be prevented. Don't worry, as now the gestational age is 33 weeks, by this time the baby might have gained the capacity to survive in extrauterine environment. Baby growth can be tracked with regular antenatal checkups and ultrasound examinations. If baby maintains normal growth, pregnancy can be continued till expected date of delivery. If IUGR increases, after reaching term, delivery can be planned. Usually IUGR babies will not have any structural defects. Take care.

Brief Answer: Bed rest, adequate hydration Detailed Answer: Hi, Sometimes baby movements can be sluggish, like when the baby is sleeping etc. Some amount of backache, leg cramps, painless abdominal tightening etc are common in pregnancy. Possibly taking rest, adequate hydration can improve them. You can use local analgesic cream to relieve backache. Vitamin supplementation especially Vitamin B12 supplementation can decrease the leg cramps to some extent. If the abdominal pain persists or increases in severity and/or associated bleeding or leaking per vaginum etc symptoms occur, go to hospital. Don't get tensed, all abdominal contractions may not indicate premature uterine action. Take care.

Brief Answer: Balanced nutritional diet Detailed Answer: Hi, Balanced nutritional diet should include cereals, dhal, milk, fruits, vegetables, meat, eggs, fish etc. Take food as three main meals and two snacks in between. Continue your medicines. Medicines containing vitamins especially folic acid and vitamin B12 helps in improving placental blood flow. Take bed rest and be peaceful. As I told loose single loop may not lead to major complications, don't worry but be careful. Hopefully you will have uneventful pregnancy with healthy baby. Take care.

Brief Answer: GTT Detailed Answer: Hi, According to studies steroid injections can increase the blood sugar levels. The effect may last for 3 to 10 days. So better to go for GTT (Glucose tolerance test) after 10 days of second injection. This will help in detecting gestational diabetes. Meanwhile make some dietary alterations like: -Avoid rice in dinner. Take foods madeup of whole wheat or wheat flour. -Avoid potatoes, pasta, fruit juices etc. -Instead go for fruits especially citrus fruits, vegetables like carrots, broccoli, spinach etc. -Take low fat dairy products. After GTT, if needed anti diabetic medicines can be started. Take care.

Brief Answer: Pressure on pelvic organs Detailed Answer: Hi, As I told previously frequency and strength of baby movements can alter from time to time. As the fetal movement count is within normal limits, no need to worry. With progressive gestational age, size of the baby will increase leading to uterine distention. This will exert pressure on pelvic organs. If there is no constant pain, no need to worry. According to the reports you posted, there is possibility of early fetoplacental insufficiency. But as I told previously the reference values can differ from machine to machine and with proper management like rest etc, placental perfussion can improve and fetal growth may progress normally. Take care.

Brief Answer: Secretions from nipples, fetal movements Detailed Answer: Hi, During pregnancy some amount of congestion and edema of the breast tissue can be seen. That can lead to feeling of heavyness and tenderness. Depending on hormonal levels these symptoms can alter. Now you watch for: -Secretions from the nipples -Fetal movements If there is no secretion from the breast and if the fetal movements are normal, no need to worry. If you are having milk like secretion from the breast and if the fetal movements are decreased, once consult gynecologist and go for ultrasound to verify the wellbeing of the baby. Take care.

Answered by Dr. Sree Gouri SR (2 hours later)
Brief Answer: Symptoms can change with progression of gestation Detailed Answer: Hi, During pregnancy vaginal discharge can increase due to increased blood supply to the local genital organs, hormonal effect etc. Gastric motility can slow down due to the effect of progesterone. That can lead to decreased appetite. In later weeks of pregnancy painless contractions can be seen. These are called XXXXXXX XXXXXXX contractions. As I told previously edema and congestion of the breast tissue can lead breast tenderness. But here it should be remembered that all these symptoms may not be same throughout the pregnancy and can differ from pregnancy to pregnancy. They can change with progression of gestational age. Intensity of symptoms like loss of appetite, nausea etc can decrease in second pregnancy compared with first pregnancy. As your baby movements are normal, no need to worry. When you consult your doctor, get examined and go for ultrasound examination to clear the doubts. Don't take tension, be peaceful. Take care.

Brief Answer: Category B drug in pregnancy Detailed Answer: Hi, Cyclopam contains dicyclomine hydrochloride. It belongs to category B drugs in pregnancy according to FDA. Means animal studies didn't show adverse effects on fetus. But sufficient human studies are lacking. In clinical practice also this is commonly prescribed. So, you can take the tablet for symptomatic relief as advised by your doctor. If the pains persist or increase in severity consult your doctor. Take care.

Brief Answer: Progesterone tablets, rest Detailed Answer: Hi, Painless contractions and mild pains are common in pregnancy. Usually they may not progress into true labor pains. If the pains become frequent and intensity increases gradually, those should be considered as true labor pains. As you are not having these symptoms, no need to worry. Progesterone tablets can help in preventing premature uterine contractions. Bed rest can help in relieving the pain. Take care.

doctor
Answered by Dr. Sree Gouri SR (14 minutes later)
Brief Answer: Usually no Detailed Answer: Hi, Usually uterus will be in slightly dextro rotated position. So sleeping in left lateral position will mobilize the uterus from main abdominal blood vessels. This will increase the blood supply to placenta. And yes, it is difficult to sleep in same position for prolonged periods. If you turn to right lateral position for breif period, that may not harm the baby. Usually ultrasound will not harm the baby as there is no radiation exposure with it. But though there is no proven evidence it is said that frequent, prolonged periods of exposure to ultrasound may affect the hearing of the baby. Usually this problem will not arise with regular antenatal ultrasound examinations. Take care.

Brief Answer: As count is normal, no need to worry Detailed Answer: Hi, Though baby may not have complete rotary movements in advanced gestational age, the intensity of the movements will definitely increase. Perception of baby movements can alter depending on the activity of the baby and concentration of the mother. When mother is involved in some other activity, she may not concentrate on baby movements. So better is calculating the baby movements during one hour after food, while lying in lateral position. As your fetal movement count is normal, no need to worry. Take care.

Answered by Dr. Sree Gouri SR (25 hours later)
Brief Answer: In three hours more than 15 Detailed Answer: Hi, As I told previously baby movements may not be the same throughout the day. Depending on the activity of the baby number of movements per hour can alter. Sometimes baby may only give flickering movements. If overall movements of the baby in one day are normal, that is good. You calculate baby movements one hour after breakfast, one hour after lunch and one hour after dinner. If all these movements collectively comes above 15, that can be considered as normal only. As the fetal heart beat is normal, no need to worry. Take care.

Brief Answer: Take care Detailed Answer: Hi, As the NST is normal, no need to worry. You can continue with twice a week NST checkups. While standing the gravid uterus will exert pressure on lower abdomen and pelvic organs. May be because of that you are feeling heavyness in lower abdomen. Increased weight can lead to ankle pain. Using foot rests during sitting, pillow under ankles while sleeping can decrease the problem to some extent. After sitting for one to one and a half hours, walk for 5 to 10 minutes. You can travel for short distances but go slowly and avoid bumpy roads. Increasing appetite can be seen with progression of gestation, it is a good sign only. Take care.

Brief Answer: Progesterone supplementation, tocolytics Detailed Answer: Hi, Progesterones can help in preventing premature uterine contractions. If uterine contractions develop, tocolytic drugs and progesterone supplementation can help in preventing premature labor to some extent. It is difficult to predict whether premature uterine contractions will develop or not. Usually in a woman without any risk factors like hypertension, gestational diabetes, poor weight gain, infections etc, the risk of preterm labor is less. Following measures can help to some extent in preventing premature uterine contractions: -Improving nutrition Zinc, magnesium, fish oil supplementation can prevent preterm labor according to few studies. -Adequate bed rest -Increasing hydration -Avoiding stress and taking psychological support with family members -Regular antenatal checkups and ultrasound examinations to detect the early signs of preterm labor. Generally in pregnancy blood sugar levels, thyroid function tests are adviced related to endocrinology. Because during pregnancy the insulin sensitivity can alter and that can lead to gestational diabetes. And thyroid hormonal requirement will increase. In infections usually TORCH panel, VDRL etc tests are adviced to detect any antepartum infections. Take care.

Brief Answer: Meconium stained liquor Detailed Answer: Hi, When baby gets stress in the form of placental insufficiency, less liquor etc, it can pass meconium. This can be identified by altered baby movements. Sometimes meconium stained liquor can come out when premature rupture of membranes occur. In ultrasound meconium stained liquor apper as particulate matter. This may not happen in all women. Normal baby movements, adequate liquor, normal NST etc indicate wellbeing of the baby. So no need to worry. Maintaining adequate hydration, nutrition and rest are important. Take care.

Brief Answer: Get examined once Detailed Answer: Hi, Constipation can be seen during pregnancy. That can be one cause for pain at anal opening. Straining during passing stools can lead to pain and burning at the anal opening. As the gestational age will progress, uterus position will change. That can lead to upper backache sometimes. If the pain is not progressing, no need to worry. Rest can help in relieving the backache. As today you are going for ultrasound, you tell your problem to your doctor and get examined. Then with her advice you can go for local analgesic cream for anal pain. If she suggests go for laxatives. Take care.

Brief Answer: Features of true labor pains Detailed Answer: Hi, Usually true labor pains will present with features like: -Uterine contractions will occur at regular intervals. Gradually the interval between contractions will decrease. -The duration and intensity of the contractions will increase. -Usually pain will be more at back and lower abdomen. And general analgesics will not work. -Associated features like cervical changes, passage of blood stained mucus through vagina etc can be seen. So, if the pain is not severe and getting relieved by rest and analgesics, no need to worry. If the pain persists better to get examined. By keeping palm on your abdomen, you can feel the uterine contractions. By this you can get some idea regarding the duration of contractions. Take care.

Brief Answer: Term gestation Detailed Answer: Hi, Progesterone is used to prevent preterm labor and birth of babies with weight less than 2.5 kgs. After 36 completed weeks, it should be considered as term pregnancy. So as you reached term and as your baby weight is within normal limits, your doctor might have advised to stop progesterone tablets. Though external supplementation is stopped, placenta will continue to produce hormones. So, no need to worry. Take care.

Brief Answer: Don't worry Detailed Answer: Hi, Usually minor hits won't cause much problem. If there is no pain, spotting etc symptoms, no need to worry. Sleeping in left lateral position is advisable. But altering the position for some time may not cause any problem. As the baby movements are normal, no need to worry. Be peaceful. Take care.

What Does "found Single Loose Loop Around Babys Neck" Indicate?

Take care.
