What Does High Creatinine Level With History Of Kidney Transplant Indicate?
Question: Hi My Mother is a kidney transplant patient since 7 years & now her creatnine level has started increasing 1.7/1.5/1.8 , so kindly suggest what is to be done .
she is under regular treatment of Nephrologist bu he is saying it is all right.

Slow rise of serum creatinine 6 yr post transplant is a very common thing. It can be because of multiple factors.
1. the new kidney provides 50 to 60 ml gfr and as a natural process we know with time the kidney function will reduce at a rate of 2 to 3 ml gfr per year.
2. control of blood pressure and sugar are important they can increase the serum creatinine.
3. Transient increases in serum creatinine can occur due to infections. i am sure your doctor must have looked into that.
4. Low grade chronic rejection causes slow damage of the transplant kidney.
5. optimization of transplant medication.
6. see urine routine and USG dopplar or the graft kidney ( again i think the treating doctor must have taken these steps).
Diet includes low phosphorus low potassium diet with Proteins in gm/day=0.7x body wt in Kg ( 50gm for a 70 Kg person). No juices less salt and mostly vegetarian food. You can discuss with a dietician about these recommendations and plan your diet.
i think your doctor is right. do not get tense over the issue but serial monitoring is important to rule out progressive rise of serum creatinine.
