What Does Hypercellular Marrow With Trilineage Maturation In A Bone Marrow Biopsy Indicate?

Posted on Fri, 19 Feb 2016
Question: Hi, I am a 45 year old female and recently had a bone marrow biopsy due to persistent leukocytosis, lymphocytosis and thrombocytosis.. I have a history of hereditary spherocytosis with a splenectomy. My results are negative for leukemia or other type of cancerous process, for which I am relieved. However, could you explain what exactly "hypercellular marrow with trilineage maturation with marrow cellularity of 70%" means and if there is any treatment I may need to seek because of it.

Brief Answer:
Nothing to worry about that.

Detailed Answer:
Hi, dear
I have gone through your question. I can understand your concern.
You have high wbc and platelets count. Your marrow is hyper cellular with trilineage maturation. It suggest that there are high number of cells in your bone marrow and all three line cells are there. Your other reports suggest that there is no atypical cells or no any malignancy so don't worry. Just be relaxed. It can be due to some infection or other cause.

You should go for repeat complete blood count and CRP. If your count comes to normal then no actions needed. If it increase further then further investigations should be done. Right now no treatment is required.
Hope I have answered your question, if you have doubt then I will be happy to answer.

I am asymptomatic. My white count, platelet and lymphocytes counts have been slowly creeping up over the course of 7 + years. My CRP is 68.24 which is high. My ANA is negative, RH factor is negative, lupus testing is negative. JAK2 is negative. Factor V is negative. My Mom had a brother who died of Leukemia at the age of 12.

Brief Answer:
No need to worry about leukemia or other malignancy.

Detailed Answer:
Hi, dear
I have gone through your question. I can understand your concern.
Your WBC count and platelet counts are slightly higher. But you have history of splenectomy. So it is most likely due to effect of splenectomy.

Moreover your other reports for leukemia or malignancy are negative. Your bone marrow is also normal. So just be relaxed. Your high CRP suggest some kind of infection or inflammation. As you say you are symptomatic so nothing to worry.

You should repeat your complete blood count after 6 to 8 weeks.
Right now no treatment is required.

Just be relaxed.
Hope I have answered your question, if you have doubt then I will be happy to answer.
