What Does "mild To Moderate Diffuse Annular Bulge With Posterocentral" Mean?
Question: What is MILD TO MODERATE DIFFUSE ANNULAR BULGE WITH POSTEROCENTRAL PROTUSION OF L5-S1 INTERVERTEBRAL CAUSING CENTRAL CANAL STENOSIS AND BILATERAL LATERAL RECESS NARROWING and MILD DIFFUSE ANNUAL BULGE OF L4-5 INTERVERTEBRAL?
Brief Answer:
This MRI is suggestive of lumbar spondylitis
Detailed Answer:
Hello,
I have studied your case and I think that you must be having back pain and leg pain. Your MRI report is suggestive of following.
1. You are having disc herniation of lumbar five and sacral one vertebra. This disc causes nerve compression and cause of pain in the leg. This can be due to weight lifting or sudden jerk in the back.
2. There is also a mild disc herniation in the L4- L5 vertebra, and this is not as advance as the previous disc disease.
I would recommend you to show it to orthopaedic surgeon and follow further advise.
thanks

Dear Dr XXXXXXX XXXXXXX XXXXXXX
Thank you very much Sir for reply.
Treatment is going on and the MRI report shows the following .....
MILD TO MODERATE DIFFUSE ANNULAR BULGE WITH POSTEROCENTRAL PROTUSION OF L5-S1 INTERVERTEBRAL DISC IS SEEN CAUSING CENTRAL CANAL STENOSIS AND BILATERAL LATERAL RECESS NARROWING.
MILD DIFFUSE ANNUAL BULGE OF L4-5 INTERVERTEBRAL DISC IS SEEN.
REST OF THE LUMBAR INTERVERTEBRAL DISCS REVEAL NO SIGNIFICANT DISC BULGE.
FACETAL JOINTS AND LIGAMENTUM FLAVI ARE NORMAL.
THE VISUALISED LOWER END OF THE CORD AND CONUS ARE NORMAL IN POSITION AND SIGNAL INTENSITY. THE PARASPINAL SOFT TISSUE APPEARS NORMAL.
THE LUMBAR CANAL DIMENSIONS AT MIDVERTEBRAL LEVEL IN MIDSAGITTAL SECTIONS ARE :
L1 : 14 mm, L2 : 13 mm, L3 : 12 mm, L4 : 11 mm, l5 : 12 mm.
THE LUMBAR CANAL DIMISIONS ARE DISC LEVEL IN MIDSAGITTAL SECTIONS ARE :
L1-2 : 13 mm, L2-3 : 13 mm, L3-4 : 10 mm, L4-5 : 11 mm, L5 : 09 mm.
MRI screening of rest of the spine reveals :
The vertebrae shows normal contour, height and marrow signal intensity.
Mild posterior disc bulge of C5-6 and D6-7 intervertebral disc is seen.
Rest of the INTERVERTEBRAL discs reveal no significant disc bulge.
The cord shows normal signal intensity.
The cervico - medullary junction is normal. The paraspinal soft tissues appear normal.
TREATMENT TAKEN :
On the basis of MRI report, treatment viz. NERVE ROOT BLOCK GIVEN AT LEVEL L4-5 & L5-S1 by the Ortho splst. and Epidural steroid give at level L3-L4 by an Anesthetist.
I understand from other such patients that they had been advised for a complete bed rest for atleast one month through which they said that they feel better.
In my case, the Dr. said, the pain should go off completely within 3 weeks of this treatment, preferably i should take complete rest with one hr. morning walk (slowly), physio exercises and of course, the medicines.
Next he said, inspite of above prescribed advises, if the pain persists, surgery is only the remedy.
I want to know whether the procedural of treatment is in a right path????????????????
Thank you very much Sir for reply.

Brief Answer:
Your treatment is on right path.
Detailed Answer:
Hello,
I have studied your case and I think your treatment is on right path. If there is relief from pain after injection then it can be left alone, if there is continue pain then you might need a surgery for spine. It is not a big surgery. Steroid injection might not be a permanent cure. Bed rest is not more required. It only make your condition more worse.
Thanks.
