my father 62 years suffers voice slurring 3-4 weeks back . he was hospitalized and MRI was don . report enclosed. during treatment he was given Dexamethasone and levepsy 500 mg twice . while giving Dexamethasone his sugar level have increased a lot. later Dexamethasone was stopped and his voice became normal . later again his voice became slurring kind. now doctor has done Elisa test .report enclosed. and given predmet 16 mg twice and forepan 40 once. hia sugar have again increased a lot. he is taking humsulin twice 15 mg and 25 mg. he is not able to sleep even. please advice. hia sugar levels are 120 fasting and later 350 after breakfast.
Steroids (Predmet) are causing high blood sugars.

From you case history I have derived that you father developed Neurocysticercosis and was given Dexamethasone. Currently he is switched to Methylprendisolone (Predmet) in the dose of 16 mg in the morning and 16 mg in the evening and he continues to have high blood sugars.
It is obvious that the high blood sugars are because of the Steroids (Methylprednisolone) which has been prescribed. Also you have written that he as difficulty in sleeping, which is again because of the steroids. Here are my solutions to the problem:
1. While taking Predmet , take the morning dose at 7:00 am and take the evening dose at 5:00pm. Do not have the steroid after 5-6 pm. This will disturb the sleep. Also taking steroids later in the evening or night is harmful for the body.
2. Discuss with your physician if you need to reduce the dose of the Predmet. Currently the dose he is receiving seems high. When the dose of Predmet is reduced, it will have to be do gradually.
3. I would advice you to take the inj HUMINSULIN-N to be taken 20 units in the morning 7:00 am with the Predmet and 20 units in the evening with the Prednmet which he will take at 5:00 pm. Take some snack 30 min after each of the injection (you are currently taking Inj HUMINSULIN 30/70, I would advice you to take inj HUMINSULIN-N instead of the the current insulin).
4. Take sugar 4 times a day with your glucometer- 3 times before meals and once at bedtime. Keep a chart for 3-4 days and you can get back to me with the chart.
5. Start T. JANUMET 50/500 mg to be taken twice a day one in the morning and in the evening. (This is for blood sugars)
6. Get a test which is known as HbA1c (which is a blood test).
7. If the dose of Predmet is changed please get back to me because the dose of insulin will also have to be changed.
Follow the advice written above and he will do fine. You can get back to me in the blood sugar chart of 3 to 4 days
