What Does My HSG Test Report Indicate?

Question: Hello Doctor,

My wife had a HSG test. It did not pain her. The report says everything seems fine. But the gynec who conducted the test said she finds swelling so she needs 2 antibiotic injections.

I do not trust that gynec because I think it was her trick so we become her patient. That is how it goes here in this part of the world atleast. At this point I am totally confused whether to trust the words of the radiologist or gynec. I need a honest opinion from you regarding the same.

Do you find any swelling at all? Please let me know. If so, will I also have to take the antibiotics along with her because mostly I read about the swelling on Google it says this is through STD but me and my wife are married for 3 years now and none of us has been unfaithful.

I understand your concern..

The HSG report is normal.. There is no swelling in the tube and both tubes have free spillage.. The radiology report is correct..

In normal prectice, when gynaecologist conduct the HSG test they do the local examination also.. If she found any vaginal infection at the time of examination, it is right at her part to give a course of antibiotics..

Moreover if there is mild infection, it will not appear in the HSG.. Prior to starting any infertility treatment, it is always better to take a course of antibiotics.. So no need to worry about the treatment..

Mild infection can occur in any person, so dont worry.. It need not be STD, it can be a fungal infection or bacterial vaginosis..

If you need my help regarding infertility please let me know your details like the duration of marriage, since how long you have been trying for pregnancy, how's the semen analysis report, age of your wife and the nature of her periods..

Are you 100% sure there is no swelling? If so, I will not go to the gynec.

We have been trying since 2 years actively in vain. We have been married since roughly 33 months. My age is 28 and wife's age is 27.

Semen analysis also is contradictory. The 1st time I did about 15 months ago the report had 55 mil count with good motility and morphology.

The last one I did about 1 1/2 month ago or so the count is very large 175 mil but motility and morphology is not good. Although Total motile count is good.

I have attached both the reports.

Here are my 2 questions:

1) Are you 100% sure there is no swelling in the HSG scan?

2) Is the semen report okayish?

I am totally confused which report to trust and which one not to because how could my sperm count increase so much over a year and yet motility and morphology get low.

Please let me know.

Thanks for writing..
I understand your concern..

The x ray picture provided shows no swelling in the tubes.. But during the process of HSG, small swelling might not have been taken in the xray. This can be seen during the process by the performing doctor.. So just from this x ray , i cant say 100% that there is no swelling..

Semen analysis report is fine.. The count and motility in both the reports are normal.. No problem in the variation in the report.. It happens at times..

I would suggest to get a follicular study for your wife to see the status of ovulation and if ovulation is not correct , we can start her on ovulation induction drugs..
