Detailed Answer:
Hello 0000xxxxxxxx,
I have gone through your query and understand your concerns.
The MRI report is suggesting involvement of white matter of brain focally and small in size.
In view of diabetes , hypertension, small vessel disease , head injury and migraine such a finding is not uncommon.
You should get some routine blood testing like vitamin B12, thyroid profile, urea creatinine, lipid profile and immune profile (ANA and ENA).
You can upload the mri images in pdf format also .
For prevention of progression and treatment you should control blood pressure, diabetes and supplement any deficiency if found in blood investigations.
You can visit and consult a neurologist for examination and detailed evaluation.
Hope you found the answer helpful.
Do get back with further queries.

Detailed Answer:
Hello dear,
The abnormalities in brain are not primary and you need to control other more important related and causative conditions like diabetes and hypertension.
You should visit and consult with a neurologist for examination and investigation as advised.
please upload images.
It doesn't seems to be any rapidly progressive disorder.

Consult a neurologist and an immunologist too
Detailed Answer:
Hello ,
The usual prognosis is good as far as risk factors are controlled well.
But in my opinion you need to consult and visit a neurologist or immunologist for detailed workup of small vessel disease as renal hypoplasia with brain changes need diagnostic evaluation.
Immunologist opinion will be helpful for diagnosis.
