Brief Answer:
You need a long term treatment and follow up.

Detailed Answer:
Hi

Thank you for asking HCM

I have gone through your query and MRI with diligence. Your problem is most likely due to lumbar spondylosis. Long term treatment with follow up is necessary for this. You can consult an orthopaedic surgeon specialised in spine for this.

NSAIDs like Aceclofenac and muscle relaxants like thiocolchicoside (zerodol TH) is helpful to control the symptoms. Neurotropic drugs like pregbalin (prega M) is also good for neurological pain syndrome also.
You should also go for physiotherapy like TENS , Massage etc. They will also teach you muscle strengthening exercises. Wearing special corsets also will be helpful. Modifying your posture while sitting and sleeping also will be advised by physiotherapist.

Most cases will be improved by conservative treatment alone.

Let me know if anything not clear. i am happy to help you.
Thanks.

Brief Answer:
You need medication and physiotherapy.

Detailed Answer:
Welcome again.

Thank you for your follow up query.

You need medication and physiotherapy. With out muscle relaxant and NSAID it will be difficult for you. If you need a long term muscle relaxant he may switch on to another one like cycloenzaprine. Also neurotropic drugs will be helpful for nerve recovery. So you should go to orthopaedic surgeon as well as physiotherapist.

Let me know if anything not clear.I am happy to help you.
Thanks.
