Not a stroke, several possibilities
Detailed Answer:
I read your question carefully and I understand your concern about your friend.
I imagine your/his main concern is a stroke given his prior history, but let me reassure you that is not the case. The MRI is the most sensitive exam to detect a stroke when present, if the MRI showed nothing it certainly isn't a stroke.
As for what could it be, there can be many origins to balance issues. Inner ear and the supplying nerve problems can manifest with balanc issues. Usually it is in the setting of vertigo, sensation of spinning movement.
Anemia is another common cause of dizziness. Metabolic alterations may also manifest with such dizziness. For that reason some routine blood tests such as complete blood count, glucose, liver and kidney function, electrolyte panel, thyroid function, vitamin B12 level are advised.
Among neurological causes oncethe head MRI results normal peripheral neuropathy is a possible cause. However I have to assume the neurologist made a thorough neurological exam and would have noticed correponding changes (I wonder what the neurologist suggested as the possible cause by the way).
