Brief Answer:
Minor herniation

Detailed Answer:
Hi,

Thank you for posting the question.
I have gone through the attached MRI in detail.

The L5-S1 disc has a tear with extrusion of disc material. This may be causing nerve compression leading to back pain radiating to the lower limbs.

L3-4 and L4-5 discs also have annular bulges with minor herniation. These discs do not seem to be compressing the thecal sac or the spinal nerve roots though.

So,in my opinion, the symptoms are majorly due to the torn L5-S1 disc.

L5-S1 disc may need decompression surgery while L3-4 and L4-5 discs can be treated conservatively by using lumbar back supports and physiotherapy / exercises.

I hope the answer was useful.

Feel free to ask me any follow up queries that you may have.

Thank you.
Kind regards.
