Question: mild cerebral involutional change? what does it mean? any connection to dementia?

Hi,

I think the above findings are from an MRI of the brain. Is it?
Let me explain you. Imagine a picture of brain as a rounded surface covered in deep ridges and grooves as that in a fold. The spaces in between these folds are called the cortical sulci. As humans age, the folded brain tissue starts to shrink, making the spaces between the folds (the sulci) deeper and wider. These are called involutional brain changes.

In many people with dementia, the folds are greatly shrunken. The involutional changes described in this report are milder, people with such reports have normal intellect and may not develop dementia quickly. Yes if it shrinks further it may develop dementia.

Regarding my uploaded report, my concern is about cerebral white matter hypodensity which may be due to microvascular ischaemia. Can this mean small vessel rupture? If this is the case, what are the implications in long term and what are the treatment options? What can be done to prevent further deterioration?

yes, microvascular ischemia is a known entity in this age group with the said changes in the MRI of brain.
If the intelligence is intact we often advise challenging brainy tasks like puzzles, sudoku, chess on a daily basis. We also advise physical advise to such patients to prevent deterioration.
There are certain non specific medications like pirocetam, nutraceuticals that can be tried.
