I am sorry for to know about what happened, but it's a relieve that he survived 100% blockage and I am quite positive that he will be improving if he takes good care.
After a heart attack, decrease of libido is a natural thing there are several reasons to it.
1) Post trauma stress.
2) Fear of exertion.
3) Anxiety.
4) Fear of a cardiac event and feeling of tachycardia during sexual activity.
5) There are some cardiac meds which can cause problems with libido specially the beta blockers.
Before I give any suggestions I would ask you to tell me hi medication in detail so that I can suggest you a plan of dealing with it. It is not just going to be a med or anything he takes to change everything it will be a whole process to return to normal life after a near death experience.
It would need meds, psychological support and better health .
Please tell me his detailed meds and also clear one thing to me what did you mean by level 25,35, and 45 , is it his ejection fraction you are talking about?

Please tell me if you think his heart damage after three months will possibly repair itself? I only know of 2 medications he takes Warfrin, Lisinopril, He takes arppoximately 6-8 medications. Has not exercised yet. If his heart attack was caused from the 100% blockage of his widow artery, could he have another heart attack, even though his blockage is now clear?

Thanks for writing back, I apologize for a delayed response , had some connectivity issues at my send. Thanks for bearing with me.
I will now answer a few questions you asked in detail.
1) Will his heart recover?
When widow maker artery or LAD gets blocked it may cause severe damage to the left ventricle of the heart which actually maintains the pump function of the heart, the blockage results in two things
1) Decreased blood supply to the muscles.
2) Death of some muscles.
The muscles who get decreased blood supply can recover but the muscles who die because of no blood supply can not recover. How much a person will recover depends upon how many muscle fibres did die. If they are more the recovery is bad if they are less in number, the recovery is good.
Please share with me , his Echo report if you can have so that I may tell you what is current condition of his heart. You used a word LEVEL 45, i am not really sure what you meant by that but if it meant ejection fraction 45% then I would say it's quite a good ejection fraction ..a normal person has ejection fraction of 60% so 45% is not bad at all for normal functioning of life.
But I would be able to give you more clear opinion if you can share is echo report.
Answering your question directly, i would say that after 3 months it is not expected to see a big recovery but his heart will recover slowly to some more extent if he takes care,,works within limits takes medicine well and regularly and doesn't exceed his limit which may cause more damage to heart.
2) His dizziness:
To comment on this , it will be great if you can share all his meds with me , I don't think it will be difficult for you to get to know what meds does he take. But still there are few things which I would like to mention which might cause dziziness in a person after a heart attack taking heart meds.
Main cause of such incidents is an arrhythmia or a fall in blood pressure specially in so called Widow maker artery blockage.
If I could know complete list of his medicines I would have been able to guide you better but still I would say he should be checking his blod pressure every now and then specially after taking blood pressure meds like Licinopril and others he might be taking,
If his blood pressure falls or he feels dizziness on getting up from lying or sitting position , he should contact his doctor immediately for dose adjustment of his medication.
3)Libido:
There are certain heart meds specially the beta blockers which have a side effect of effecting potency, and beta blockers are given to almost every one after heart attack, so if the has problems with potency his meds , specially the beta blockers can be a cause, He can discuss with his doctor to get alternate meds than beta blockers.
Libido loss is also caused by anxiety related to heart problems so he should discuss everything with his doctor and ask him about his sexual activity and how much active he can be,,
when his doctor will tell him that he is safe for that and he can be involved in activity then automatically his fear and anxiety will get relieved and that will make things easier and better for him.

It's ok if he is not on an exercise program his doctor must know this better, sometimes patients who are physically active any ways like your friend is, they don't need a special exercise program that all depends upon what their doctor decides in their best interest.
Yes MRI so some types can tell about heart damage I can elaborate it to you if you can share it's report with me.
If your friend was having these unconsciousness problems before heart attack then it might be a brain issue but if that started to happen for the first time after his attack then it would be suspected that there was some problem related to his heart or his meds.
I will wait for you to come up with his reports and list of medicines so that i can guide you in a better way.
