What Does My MRI Scan Test Report Indicate?

Question: My father ,56 years old ,felt some spin in the head two months back. After repeated spin problem MRI test was done and he has been diagnosed with AVM of the size of about 3.6x2.5 cm in the parieto occipital region of the brain. After doing MRI and DSA test ,the doctors at appollo XXXXXXX has suggested Embolization in 3 to 4 stages then radiosurgery followed by radiosurgery if required. Whereas doctor in Fortis XXXXXXX are suggesting radiosurgery first and surgery at later stage ,if required but the time period of interval between two radiosurgery is one year and the problem of any rupture of vein during this intermittent period may be dangerous. Please advise what to do..we are highly confused n scared too about which process need to be followed....? Pl note that he has never faced any problem till this age and he has also undergone one operation in lest ear 6 years back and has some hearing problem.

Hello dear,
I have gone through your question and understand your concerns.
The treatment for AV malformations depends upon many factors like size, location, feeding arteries, draining veins, previous hemorrhage, deficit.
The decision regarding mode of treatment depends upon grading based on spetzler XXXXXXX criteria.
After reviewing all reports the best approach would be to go for presurgical embolisation leading to decreased in vascularity and nidus of AVM. Then radiosurgery will lead to occlusion of vessels and obliteration of nidus. Surgical removal if required can be done at later stage. This approach will be better as there are many feeders to nidus and will be difficult to get good radio surgery related vessels occlusion.
Go with Apollo approach.
