Hello
I have gone through the medical details provided by you of your father and i understand your concern. I would try to explain you the matter in simple way.
TMT is basically a stress test which is done to see the adequacy of blood supply to the heart muscles during increased work load or physical activity. When a person start running or exercising the workload of the heart increase so do the blood supply demand of the heart muscles, now when there is some kind of disease in the heart arteries then the amount of blood supply decreases causing ischemia to the heart muscles which are shown as ECG changes during TMT.
As your father is a diabetic as well as 60 years these condition make him more prone for a heart problem (esp. diabetes).
So YES its advisable to go for an ANGIOGRAPHY to see the status of this heart vessels. I would also like to advise to go for an ECHO also to see the heart function.
If he does not feel any kind of chest pain or discomfort or shortness of breath while working there is no urgency but yes he should go for it as early possible.

Thanks for your reply. It was helpful though, I also like to inform you that his blood sugar level is elevated for past 6 months inspite of taking continued diabetic medications. So as I personally feel it could be because of this high blood sugar; So from the below options I seek your advise to know which should be our first move :
Get admitted and bring the blood sugar into control
Go in for a bone specialist as he feels vertigo
Go for an angiography.
