What Does My Blood Test Report Indicate?
I am 32 years old. Last year I had a prolonged period for 3 weeks. The doctor has done blood test. Below is the result for hormone test:
Thx level 5. T3 and t4 are normal
Hemoglobin 14.
PCOS- FSH LEVEL 14
They called this as 'Subclinical Hypothyroidism'. I was not given any medicine for this.
Had done ultrasound, uterus biopsy. Have been diagnosed with Simple Hyperplasia. For which, I have been given MedoxyProgesterone (Meprate 10mg). I have been taking this for an year now from 15th day to 24th day in my mensuration cycle. After taking the tablets for six months, a second biopsy was done and the results were normal. But even then they asked me to take Meprate, to regularize my period. The recent thyroid test shown the results as 5.86
For two cycles, i got spottings and break through bleeding even when I am taking the tablets. Now my question is, do I need to take any medication for Subclinical Hypothyroidism?. Is it safe to continue Meprate?. Please let me know.
It's better to treat Hypothyroidism,Meprate can be for 1 1/2 to 2 yrs

Welcome to HCM. I had gone through your question and understand your concerns.
I will explain both your questions.
1. Sub clinical hypothyroidism
The treatment of this condition is controversial. Though there is benefits of treating this condition in most patients, the definite indications of treatment are any one of the following
a. Presence of thyroid swelling
b. Increased antithyroid antibodies (anti thyroid peroxidase and anti thyroglobulin antibodies)
c. Elevated cholesterol levels in blood
d. Pregnancy or planning for pregnancy.
Since you have menstrual irregularities and hypothyroidism is one of the causes of menstrual irregularities, if I were your treating endocrinologist I would have started you on medications. The treatment with a small dose of thyroxine is not of any side effects.
2. Hormonal therapy in PCOS
The treatment of PCOS is with estrogen progesterone pills (Contraceptive pills). Usual treatment is combine oral pills (OCPs) for 1st 3 weeks of menstrual cycle. Some doctors prefer treatment with progesterone alone (meprate) from 15-24 th day of cycles. There is no treatment guideline for duration of treatment. If there are no side effects the usual treatment duration is 1 1/2 to 2 years. Oral pills are associated with side effects - like breast pain, irritability, mood disorders, migraine episodes etc.

It is normal to have in between bleeding

Welcome back.
This type of intermittent bleeding or spotting can occur with treatment with meprate.
Two hormone from your ovaries has to work in harmony to have regular cycles. First the esterogen increase the thickness of lining of uterine cavity. Then progesterone make it mature and once that hormone is levels decrease the lining sheds as menstruation.
In PCOS both the hormones are not properly produced. In your case what the doctor is trying is to shed the lining every month using progesterone withdrawal. The timing and duration of bleeding depends on the interplay between natural hormones produced by your ovaries and the progesterone you take as meprate. There won't be a perfect harmony between these two leading to irregular spotting while on drugs.
