What Does My Complete Health Check-up Result Report Indicate?

Question: My father had vertigo in Nov'16. Post which we got the complete health check up done. In health check up we came to know that his heart is pumping at 50-55% and b12 is below 100, also kidney functioning has deteriorated. RBC, WBC, PSV is low and platelet count is around 85000. Doc gave him 10nos of b12 injections and medicine for kidney. We also ruled out Dengue by test. Hematologist did PT, PTT, S.Ferritin, HIV, post which she suggested FISH for MDS, cytogenetics and Bone marrow biopsy. I have attached report for these here. Please advise what shall I do next? Or any diagnosis that you can conclude on?

I have gone through your question.
I can understand your concern.

You have pancytopenia.

Your bone marrow biopsy report suggest that you have primitive or blast cells. It can be seen in myelodysplastic syndroma, leukemia or myeloproliferative disease.

Your other reports are normal. Your cytogenetics report is also normal. So now for diagnosis immunophenotyping study is required.

Consult your treating doctor and plan accordingly.

Iron deficiency is unlikely.

Detailed Answer:
Hi, dear
I have gone through your question.
I can understand your concern.

You have pancytopenia with atypical primitive cells.

Iron deficiency cause low hemoglobin but it is not related with low wbc and platelets.Moreover in iron deficiency MCV is low but in your case MCV is high so it's unlikely to have iron deficiency.

Folic acid deficiency can leads to pancytopenia and high MCV. But again atypical and primitive cells in bone marrow is not correlate with that.
Still you should check it once because you may have folic acid acid deficiency along with some other pathology.

But overall you have other major disease and we have to find it.
