default
Posted on Tue, 12 Jul 2016
Question: Helo sir, i have met nearly all top cardiologist in XXXXXXX .actually there is non specific change in my tmt..i have gone through all test like holter, ecg, tmt, stress echo and i have even acheived 100% of target heart rate with no pain ,no breathing problem etc.all doctor said am absolutely healthy..but i got depressed just to hear that non specific change in tmt..for this i pressurized doctor to do my CAG for my clearification..while doing CAG doctor say you are perfectly okay..its like a baby heart..all is good but i still get depressed by remembring that non specific change..what should do
I swim thrice in a week and do some yoga randomly..sir my bp is normal but sometimes get lil higher @135/84
And my pulse is always in between 55 to 80 and in the night it most of the time shows around 54 to 66..does ST depression inversion or t wave changes mean that i have heart problem or indicating any type of prob like pace maker etc..please satisfy my anxiety n negative thoughts.
Sir non specific t-wave abnorrmilities..i have uploaded my stress echo report kindly look at it patiencly.

Detailed Answer:
Hello!

Thank you for asking on HCM!

I carefully reviewed your uploaded cardiac test reports and would explain that
your stress echocardiography seems to be perfectly normal (there is no inducible ischemic abnormality in myocardial kinetics.

So you should relax because those nonspecific ECG changes during stress test do not correlate to any cardiac ischemic events.

Regarding those ECG changes, there are some upsloping ST depressions, which have no clinical significance regarding any myocardial ischemia. They are frequently found during sinus tachycardia on physical exertion.

So, coming to this point, there is no any medical evidence of cardiac ischemia in your cardiac tests.

In addition, I would like to explain that for screening your coronary risk for potential future cardiac events (especially if you have any coronary risk factors present, like hypertension, dyslipidemia, smoking contacts, heredity for CAD or glucose metabolism abnormalities), I would recommend a coronary angio CT.

This would definitely clarify this issue and relieve your anxiety.

It would also clarify the significance of those non specific T wave abnormalities.

Meanwhile, try to relax your mind and keep following a healthy lifestyle (healthy diet, a lot of physical activity).

Hope to have been helpful!

Feel free to ask any other questions whenever you need!

Kind regards,

Dr. Iliri

Brief Answer:
Opinion as follows:

Detailed Answer:
Hello again,

I would like to congratulate you about your perfect daily regimen and your interest on health issues.

As you haven’t any coronary risk factors, and your coronary angio CT has revealed normal coronary arteries, you shouldn’t be afraid of those non-specific ST up-sloping depressions.

I couldn’t find your Holter report in the uploaded file (there were only exercise stress test and respective ECGs, exercise stress echocardiography).

Nevertheless, regarding heart rate variability, I would explain that during the night hours, because of parasympathetic nervous system predominance, it is normal to have sinus bradycardia (even 30-40 bpm).

This doesn’t mean that you suffer from any cardiac conductance disturbances; no cardiac abnormalities are responsible for such a heart rate reaction.

It is just a normal physiological adaptation.

No any dangerous consequences ensue from that low heart rate during sleeping.

So, no need for pacemaker therapy is intended.

You had a perfect heart rate during exercise stress test, so any cardiac conductance disturbances (including sick sinus syndrome or isolated atrio-ventricular block) is excluded.

At the end, I would like to assure you that nothing abnormal could be concluded by your already performed cardiac tests.

The only thing to do is just take a deep breath and relax.

You are on the right way of dealing with your heart and enjoying a healthy long life.

Wishing you a nice day!

Regards,

Dr. Iliri

Oh am sorry..i am really confused now..this is last question from me..when there were non specific changes like ST depression..i didnt know what further test should i go for..i guess it was CAG..and i even got a CD of that filming and report shows that all coroneries all normal..i was very happy to see that reports..but as you said coronery angio CT..i m confused if CAG is not realiable..they insert a wire(i dont know what was that) near my vaist to the heart and images were taken and i was lying on a table..machine was moving to take images..are you saying i should go for another test as i am very much scared of these procedure..was CAG not perfect test for me.

Brief Answer:
You do not need to perform any other investigations.

Detailed Answer:
Hello again!

Now that you have prescribed the underlying procedure, I believe that CAG stands for coronarography, which is the gold standard for investigating coronary artery disease.

If coronary stenosis are excluded by this test, then this is the most exact conclusion that confirms that your coronary arteries are normal.

Coming to this point, I am glad to confirm you don’t need any further test for exploring your coronary arteries.

In fact, it was a misunderstanding, and I didn’t consider CAG for coronarography because there was no uploaded report for this test.

But now that everything is clear, I would assure you that you don’t need a coronary angio CT for this purpose.

You are perfectly normal. You have perfectly normal coronary arteries.

No further clinical investigations are necessary. Just follow your healthy lifestyle.

Best wishes,

Dr. Iliri

Brief Answer:
You are free of any cardiac disorders, including coronary artery disease

Detailed Answer:
Hello again!

I understand your concern, and would explain that the fact that coronary stenosis is excluded means that you have no coronary artery disease.

So, to conclude, your symptoms are not related to any cardiac disorders, including myocardial ischemia (coronary artery disease).

You have nothing to worry about that!

CAG is a better test than coronary angio CT for the examination of coronary arteries, because it evaluates directly the blood flow in them. It is actually the best test for the diagnosis of this disorder. No further tests are needed.

Coming to this point, I would recommend consulting with a specialist of anxiety (psychiatrist, psycotherapist) to help you manage your anxiety, which remains your main problem.

Meditation and yoga can be really useful in this situation.

I would also recommend performing a lot of physical activity (aerobics, recreational sports, etc.), which can be really helpful in the management of anxiety and your fears.

You should try to stop yourself from thinking about this issue any more!

You are a perfectly healthy person! I can assure you about that!

Hope you will find this answer helpful!

Best wishes,

Dr. Iliri
