What Does My Kidney Function Test Report Indicate?
Treatment depends upon the cause
Detailed Answer:
Hi! Welcome to health care magic!
GFR or glomerular filtration rate is on a continously decline with respect to age with an estimated 1% decrease per year since birth, however such decline in your case seems pathological and indicates towards chronic kidney decrease. But before that, difference between acute and chronic renal disease needs to be ruled out. Acute kidney injury that occurs with decreased hydration or increased water loss, as in cases of prolonged dehydration secondary to profuse vomiting or diarrhea as well as nephro toxic drugs and other kidney allergens. Once acute causes are ruled out and suspicion of chronic renal disease is established next steps to proceed will be discussed below.
This is not an entity which can be easily ignored. You need proper workup for the cause in such cases. To start with 24 hour proteinuria or spot protein creatinine ratio and ultrasound KUB are first line of investigations which are a must. If ultrasound shows parenchymal ehogenecity to be normal or slightly decreased with corticomedulary differentiation still preserved, that calls for a need of urgent renal biopsy for histopathology and case identification because it can be some underlying glomerulonephritis. I know I have flooded you with some heavy medical terms, but that all needs to be discussed with your GP. Prepharably get a referal to a nephrologist and discuss all this in detail.
Once the cause is established, treatment with steroids or immunosuppressive therapy is indicated.
If cause is established as drug induced or any ATN or interstitial nephritis, removak of offending drug, hydration and IV fluids are helpful to increase GFR.
All this needs to be discussed with your GP as soon as possible. Meanwhile keep good hydration, and repeat tests after a week to monitor the trends alongwith calcium, phosphate, hemoglobin levels alongwith iron and TiBC. All these can be affected in chronic renal pathologies.
Hoping you a speedy recovery. If you dont have any more questions, please close the discussion. You can discuss the follow ups after nephrologist review later, directly through my profile if you feel so. Regards.
