Brief Answer:
Please consider these guidelines

Detailed Answer:
Hello
Welcome again and thanks for the details
MRI report is suggestive of nerve compression due to disc protrusion.
Since you have chronic problem for 4 years and you have tried ayurveda and homeopathy everything, that will not going to help you.
In my opinion you should consult a neurosurgeon in your own according to your convenience for a corrective surgery and regular physiotherapy thereafter.

Meanwhile please follow these guidelines
Avoid lifting heavy weight
Sudden Bending forward with jerk
use hard bed to sleep
Correct your posture and gait while sitting in chair and walking.
Multivitamin and calcium supplements

As far as knee and ankle is concerned you should use knee brace and ankle brace, get an X ray of the knee and ankle to rule out any bony abnormality.

Hope this answers your question
Please feel free to ask for further queries I will be happy to help you
Best wishes

Brief Answer:
Please consider these guidelines

Detailed Answer:
Hello
Welcome again
Life style modifications are required to prevent further damage to the spine and deterioration of the pain.

Corrective surgery means extrusion at L5-S1 Level can be repaired and compression of nerve root to be relieved.
Looking at your age and clinical condition I am sure you will definitely get good results after surgery by a skilled neurosurgeon.

Surgeries are available like leminectomy, hemi/partial leminectomy or micodissectomy about which your doctor knows and best suitable to you can performed

You need to follow life style changes and have to be more cautious and careful to avoid further damage.
Stretching exercises will definitely help you in present condition.

Hope this answers your question
Please feel free to ask for further queries I will be happy to help you
Wishing you good health.
