Question: Early Morning Symptoms :burping ,severe lower abdominal pain on both sides ,sometimes burning sensation in the buttocks

Current medication :gelusil syrup ,ocid qrs 20(1-0-1),LIBRIUM 10MG (0-0-1)

I am done my endoscopy scan .could you please tell the cause of the issue ,interpret the report for me as pain is till there .could you suggest some medication as my current medication is not helping me reduce my abdominal pain .

Thanks for your query and an elucidate history.
To recapitulate: Male/*30 - endoscopy on 5 XXXXXXX 2016 shows duodenitis only - C/O early morning burping, severe **lower abdominal pain on both sides - sometimes burning sensation in buttocks - taking Gelusil, Ocid qrs and Librium at night - wants to know the cause and suggestion for medicines as the present treatment is not helping to reduce the abdominal pain...

Please give me some more information so that I can assist you better:
- Since when have you got the symptoms?
- What is the nature of your work/job?
- What is the amount of stress and/or anxiety?
- What are the factors like food, milk, tensions, worries that increase your pains?
- Which things reduce or control your pain?
- How are the stools- color, consistency, smell, mucus, blood or black?
- Any other information that you would like to share, albeit personal that may be related to the present problem.

From the short history you have provided, the cause of the lower abdominal pain can be IBS (irritable bowel syndrome), colitis or so.

I would advise you the following:
- Tests of blood: the routine, thyroid, liver and kidney functions.
- Tests of urine, tests of stool for 3 consecutive days.
- Ultrasonography of the abdomen and preferably CT scan of the abdomen.
- Colonoscopy if indicated.

All these tests are necessary to find out the causes and treat them accordingly.
If these tests are normal, your problem can be labelled as IBS and treated accordingly.
Make a list of all the foods and beverages and other factors that instigate or enhance your problems and to stop them.
Control of stress and/or anxiety. This is one of the most important part of the treatment.
Discuss with your Doctor to stop Librium and to start Librax as this contains antispasmodic medicine also and to be taken as per the requirements.
Treatment as per the specific causes as found on clinical examination and on investigations.

Duodenitis can be a part of the IBS or usually occurs in patients with lots of stress and anxiety.
The same medicines will help to get duodenitis under control

since 3 months
- What is the nature of your work/job?
Software engineer work time between 11am to 8pm plus travel time of 4 hours to and fro by cab
- What is the amount of stress and/or anxiety?
normal
- What are the factors like food, milk, tensions, worries that increase your pains?
pain occurs early in the morning only .Day is normal without any pain
- Which things reduce or control your pain?
hot water with lime and honey or milk. Pain subsides after half hour of waking up even if i dont take any of the above .
- How are the stools- color, consistency, smell, mucus, blood or black?
Normal no change before and after this issue
- Any other information that you would like to share, albeit personal that may be related to the present problem.
In terms food my habits i eat :Chapati/Parathas with curd,chicken fried and Biryani once a week,eggs ,idly,dosa,rice only over the weekend ,milkshakes ,tea 3-4 times a day

Thanks for a very systematic follow-up.
This says that your have pain for half an hour in the morning whether you take something or not.

Ultrasound says normal study.
You may please take Gelusil and Ocid qrs the moment you wake-up and see the results.
Keep away from fried things and hot and spicy things.
No aerated drinks and hold/reduce tea for a few days.

Rest of the things as we already discussed above.

Hello Doctor,

Thanks for your advice. Could you please let me know how long should I be taking the Ocid qrs and gelusil.

Is PAN D the same as OCID qrs ? Which would you prefer doctor. Thanks

Pan-D once a day.

Detailed Answer:
Take Gelusil and Ocid qrs for about a month ans is meant for duodenitis.
Pan-D is again a PPI called pantoprazole but has domperidone with it.
Hence Pan-D will help for burping too.
Pan-D is to be taken once daily only, the moment you wake up.

For abdominal pain you have to undergo the tests and medication as we discussed above.
