Question: Can you please review the pictures attached, I've been previously diagnosed with a mild form of HPV, I've undergone two laser procedures to have these lesions removed but have unfortunately come back. I also have PCOS, not sure if that's relevant. Not currently taking any medications to help with this issue. Is there any medication I can take to help aid in treating my issue?

Thanks for the query to HCM,

I have seen the pictures attached and these are genital warts covering vulva, vagina and anal regions in your case.

In most patients, treatment can induce wart-free periods. If left untreated, visible genital warts can resolve on their own, remain unchanged, or increase in size or number. Available therapies for genital warts likely reduce, but probably do not eradicate, HPV infectivity. Whether the reduction in HPV viral DNA resulting from treatment reduces future transmission remains unclear. No evidence indicates that the presence of genital warts or their treatment is associated with the development of cervical cancer.

There are various regimens available for treatment of genital warts:- you have already undergone laser and now they have reoccurred. The different medications used are:_

Patient-Applied:

Podofilox 0.5% solution or gel OR Imiquimod 5% cream OR Sinecatechins 15% ointment

Provider–Administered:

Cryotherapy with liquid nitrogen or cryoprobe. Repeat applications every 1–2 weeks.
OR Podophyllin resin 10%–25% in a compound tincture of benzoin OR
Trichloroacetic acid (TCA) or Bichloroacetic acid (BCA) 80%–90%
OR Surgical removal either by tangential scissor excision, tangential shave excision, curettage, or electrosurgery.

You may opt for PODOFILOX application and see the difference. Podofilox is an antimitotic drug that destroys warts, is relatively inexpensive, easy to use, safe, and self-applied. Podofilox solution should be applied with a cotton swab, or podofilox gel with a finger, to visible genital warts twice a day for 3 days, followed by 4 days of no therapy. This cycle can be repeated, as necessary, for up to four cycles.
If this doesn't help then you may have to opt either for cryotherapy or for a repeat laser.

Thank you Dr. XXXXXXX . last questions.. Would you be able to tell me what is causing the discoloration in my genital region.. viewing the first picture you can see it's like there's a ring of discoloration (darker) over all my genital area.. also can you advise what I can do to great it?

Hello,
The ring along the genital area with red well demarcated borders and possibly itchy throughout, indicates that its a ring worm infection or TINEA infection. This shall need a local antifungal ointment like OINTMENT CLOTRIMAZOLE 1% twice a day application for 7 days along with Tab FLUCONAZOLE 300mg once a week x 3 weeks.
This should help curtail the spread of TINEA further. Try to keep the local area moisture free and dry as much as possible.

Thanks again Dr. XXXXXXX I also suffer from chronic yeast infections.. so I've become immune to fluconazole.. is there another medication you can recommend I use to treat the tinea as well as the chronic yeast infections?

In that case , you can use orally - CAP ITROCONAZOLE 200mg x once a day x 5 days.
Review after 5 days with pictures for local area to see the effect and then a repeat course may be suggested if required depending on improvement.
