What Does My Lab Test Report Indicate?

Question: Hello Doctor,

My mother, aged 56 has been complaining of the pain in bones. We visited the local hospital and doctor prescribed the spine DXA scan for which results are attached.
The T-scores are negative in all the cases as shown. Doctor is saying the bones have gone very weak and prescribed the following supplements :

1. Two vitamin D shots in last 15 days (already given)
2. Calcium supplements. (continuing)

Further following tests have been prescribed:
1. Blood urea
2. Blood sugar pp
3. Blood creatinine

Depending on the test results he is saying some injection(shot) might be prescribed to stop calcium from withering away from the bones.

I am not really sure about the side effects of that injection(shot). My mother had her menopause 4 years back.

Need second opinion on the scan results and the medication prescribed. Is there any other precautions we should take?

Hi , I am Dr Gopal Goel Orthopedic Surgeon.

Your mother seems to be suffering from osteoporosis in the early stage as per the dexa report, BMD around -2.5. She needs regular medication of Vit D3 and calcium supplements. In my practice, I give oral medications only and reserve injections only for severe osteoporosis and for people who cannot tolerate some oral medication.

The injection your doctor is planning is perhaps zoledronic acid which is to be given once a year . Some precautions are required in giving the injection, else apart from fever for 2-4 days no other major side effect is expected.

Thanks a lot doctor for your answer. Regarding the "Some precautions are required in giving the injection", what exactly this precautions are? Are they from patient or from doctor's side?

Brief Answer:
Creatinine clearance and well hydrated patient.

Detailed Answer:
For this injection , patients creatinine clearance( Blood urea and S Creatinine are being advised in your case) should be normal. Patient should be well hydrated like have taken at least one liter of liquids before injection. Normally I give one bottle of saline before giving this injection. This injection is to be given very slowly in about an hour.
